外周血NLR、PLR、CRP与维持性血液透析患者发生导管相关性血流感染的关系研究

摘　　要：目的探讨外周血中性粒细胞计数与淋巴细胞计数比值(NLR)、血小板计数与淋巴细胞计数比值(PLR)、C反应蛋白(CRP)与维持性血液透析(MHD)患者发生导管相关血流感染(CRBSI)的关系。方法回顾性选取2011年3月至2021年12月在安吉县人民医院接受MHD治疗的100例患者为研究对象,根据病理学诊断结果提示是否发生CRBSI分为感染组28例和未感染组72例,比较两组患者临床资料,采用多因素logistic回归分析MHD患者发生CRBSI的影响因素。结果两组患者在年龄、MHD病因、管型、置管时间、白细胞计数、嗜酸性粒细胞比例、CRP、NLR、PLR等方面比较,差异均有统计学意义(均P<0.05)。置管时间(OR=1.097)、CRP(OR=1.019)、NLR(OR=1.255)均是MHD患者并发CRBSI的独立影响因素(均P<0.05)。结论对于外周血NLR、CRP水平较高以及置管时间较长的MHD患者,应警惕CRBSI的发生。Objective To investigate the significance of peripheral blood neutrophil count to lymphocyte count ratio(NLR),platelet count to lymphocyte count ratio(PLR)and C-reactive protein(CRP)in maintenance hemodialysis(MHD)patients with catheter-related blood stream infection(CRBSI).Methods This study retrospectively reviewed 100 patients who underwent MHD in Anji County People's Hospital from March 2011 to December 2021.The patients were divided into infected group(28 cases)and uninfected group(72 cases)according the presence of CRBSI.The clinical data of the two groups was collected to screen the independent risk factors for CRBSI in MHD patients by using multivariate logistic regression analysis.Results There were statistically significant differences between the two groups in age,MHD etiology,tube type,catheter time,white blood cell count,eosinophilic ratio,CRP,NLR,and PLR(all P<0.05).Multivariate logistic regression analysis showed that longer catheterization time(OR=1.097),high peripheral blood CRP(OR=1.019)and NLR(OR=1.255)were independent risk factors for CRBSI in MHD patients(all P<0.05).Conclusion For MHD patients with high levels of peripheral blood NLR and CRP and long catheterization time,the occurrence of CRBSI should be vigilant.
