摘　　要：如何以家庭医生作为抓手,提升慢性病管理的水平仍然是我国的一大难题。本研究总结了目前我国已有的慢性病管理模式、家庭医生签约服务模式、新型数字健康技术,阐述了三者之间的联系,并探讨了新型数字健康技术在慢性病管理和家庭医生签约服务中的优势,以期为提升慢性病管理水平提供坚实的理论基础。How to use family doctors as a starting point to improve the level of chronic disease management is still a major challenge in China.This study summarizes the current chronic disease management models,family doctor contract service models,and new digital health technologies in China.It expounds the connection between the three and explores the advantages of new digital health technologies in chronic disease management and family doctor contract services,aiming to provide a solid theoretical foundation for improving the level of chronic disease management.
