摘 要:目的基于国家健康与营养调查(NHANES)数据探讨体重调整腰围指数(WWI)与高血压的关系。方法本研究在NHANES数据库中选取国家健康统计中心(NCHS)2017—2023年的调查对象13319例。收集调查对象的临床资料,计算WWI。按照调查对象WWI的四分位数将其分为Q1(WWI≤7.12)组、Q2(WWI为7.13~8.06)组、Q3(WWI为8.07~9.14)组、Q4(WWI>9.14)组。采用多因素Logistic回归分析探讨WWI与高血压的关系,绘制平滑曲线分析WWI与高血压的相关性,并进行对数似然比检验。结果所有调查对象中,WWI≤7.12者3330例、7.13~8.06者3329例、8.07~9.14者3330例、>9.14者3330例。四组性别、年龄、种族、贫困收入比(PIR)、受教育程度、吸烟状况、SBP、DBP、高血压发生率、糖尿病发生率、腰围、体质量、BMI、总胆固醇、糖化血红蛋白、空腹血糖比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析模型1、2、3结果均显示,WWI与高血压呈正相关(P<0.05)。平滑曲线分析结果显示,WWI与高血压呈非线性相关,当WWI>7.44时,高血压患病风险增加26%〔95%CI(21%~31%)〕。对数似然比检验结果显示,WWI对高血压有显著影响(P=0.003)。亚组分析结果显示,WWI与高血压的关系在性别、年龄、PIR上存在交互作用(P<0.05),其在种族、是否吸烟、是否发生糖尿病上无交互作用(P>0.05)。结论基于NHANES数据发现,WWI与高血压呈非线性相关,当WWI>7.44时,高血压患病风险增高26%,尤其在男性、年龄≤60岁及PIR为3.77~5.00的人群中,其关联性更强。Objective To analyse the relationship between weight-adjusted waist index(WWI)and hypertension based on National Health and Nutrition Examination Survey(NHANES)data.Methods A total of 13319 respondents were selected in National Center for Health Statistics(NCHS)from 2017 to 2023 in NHANES database.The clinical data of respondents were collected,and WWI was calculated.The respondents were divided into Q1(WWI≤7.12)group,Q2(WWI was 7.13-8.06)group,Q3(WWI was 8.07-9.14)group and Q4(WWI>9.14)group according to quartiles of WWI.Multivariate Logistic regression analysis was adopted and smooth curve was drawn to analyze the relationship between WWI and hypertension.And log-likelihood ratio test was performed.Results Among all the respondents,there were 3330 cases with WWI≤7.12,3329 cases with 7.13≤WWI≤8.06,3330 cases with 8.07≤WWI≤9.14,and 3330 cases with WWI>9.14.There were significant differences of gender,age,race,poverty income ratio(PIR),degree of education,smoking,SBP,DBP,incidence of hypertension,incidence of diabetes,waist circumference,body mass,BMI,total cholesterol,glycated hemoglobin,fasting blood glucose among the four groups(P<0.05).Multivariate Logistic regression analysis model of 1,2,3 results showed that WWI was positively correlated with hypertension(P<0.05).Smooth curve analysis results showed a nonlinear correlation between WWI and hypertension.When WWI was greater than 7.44,the risk of hypertension increased by 26%[95%CI(21%-31%)].The results of the log-likelihood ratio test showed that WWI had a significant effect on hypertension(P=0.003).Subgroup analysis result indicated that there was an interaction effect of relationship between WWI and hypertension on gender,age and PIR(P<0.05),and there was no interaction effect of relationship between WWI and hypertension on race,smoking status,or diabetes(P>0.05).Conclusion Based on the NHANES data,WWI is found to have a non-linear association with hypertension,with an increased risk of hypertension when WWI>7.44,especially in male,population wi
