摘　　要：目的探讨心脏电生理平衡指数(iCEB)与老年冠心病合并高血压患者同型半胱氨酸(Hcy)及SYNTAXⅡ评分的关系。方法选取2023年度首都医科大学附属北京康复医院收治的老年冠心病合并高血压患者129例为研究对象。收集患者的临床资料,计算iCEB,根据Hcy将患者分为H型高血压组(Hcy≥10μmol/L,n=97)及非H型高血压组(Hcy<10μmol/L,n=32),根据SYNTAXⅡ评分将患者分为高分组(SYNTAXⅡ评分≥32分,n=45)、低分组(SYNTAXⅡ评分<32分,n=84)。iCEB与老年冠心病合并高血压患者Hcy、SYNTAXⅡ评分的相关性分析采用Pearson相关分析;老年冠心病合并高血压患者Hcy、SYNTAXⅡ评分的影响因素分析采用多重线性回归分析。结果H型高血压组与非H型高血压组舒张压、收缩压、空腹血糖(FBG)、糖化血红蛋白(HbA1c)、iCEB、SYNTAXⅡ评分比较,差异有统计学意义(P<0.05)。低分组与高分组FBG、iCEB、Hcy比较,差异有统计学意义(P<0.05)。Pearson相关分析结果显示,iCEB与老年冠心病合并高血压患者Hcy、SYNTAXⅡ评分呈正相关(P<0.05)。多重线性回归分析结果显示,舒张压〔β=0.127,95%CI(0.038~0.216)〕、收缩压〔β=0.085,95%CI(0.022~0.148)〕、高密度脂蛋白胆固醇(HDL-C)〔β=-1.872,95%CI(-3.237~-0.507)〕、FBG〔β=0.463,95%CI(0.155~0.771)〕、HbA1c〔β=0.917,95%CI(0.356~1.478)〕、iCEB〔β=3.217,95%CI(2.268~4.166)〕、SYNTAXⅡ评分〔β=0.348,95%CI(0.216~0.480)〕是老年冠心病合并高血压患者Hcy的独立影响因素(P<0.05);BMI〔β=0.034,95%CI(0.010~0.058)〕、FBG〔β=0.045,95%CI(0.021~0.069)〕、HbA1c〔β=0.023,95%CI(0.003~0.043)〕、iCEB〔β=0.345,95%CI(0.256~0.434)〕、Hcy〔β=0.025,95%CI(0.005~0.049)〕是老年冠心病合并高血压患者SYNTAXⅡ评分的独立影响因素(P<0.05)。结论iCEB与老年冠心病合并高血压患者Hcy及SYNTAXⅡ评分呈正相关,iCEB是老年冠心病合并高血压患者Hcy及SYNTAXⅡ评分的影响因素。Objective To investigate the relationship between cardiac electrophysiological balance index(iCEB)and homocysteine(Hcy)and SYNTAXⅡ score in elderly patients with coronary heart disease combined with hypertension.Methods A total of 129 elderly patients with coronary heart disease combined with hypertension admitted to Beijing Rehabilitation Hospital,Capital Medical University in 2023 were selected as the research subjects.Clinical data of the patients were collected,and iCEB was calculated.Patients were divided into H-type hypertension group(Hcy≥10μmol/L,n=97)and non-H-type hypertension group(Hcy<10μmol/L,n=32)based on Hcy.Patients were divided into high score group(SYNTAXⅡ score≥32,n=45)and low score group(SYNTAXⅡ score<32,n=84)according to SYNTAXⅡ score.Pearson correlation analysis was used to explore the correlations between iCEB and Hcy,SYNTAXⅡ score in elderly patients with coronary heart disease combined with hypertension.Multivariate linear regression analysis was used to explore the influencing factors of Hcy and SYNTAXⅡ score in elderly patients with coronary heart disease combined with hypertension.Results There were significant differences in diastolic blood pressure,systolic blood pressure,fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),iCEB,and SYNTAXⅡ score between H-type hypertension group and non-H-type hypertension group(P<0.05).There were significant differences in FBG,iCEB,and Hcy between high score group and low score group(P<0.05).Pearson correlation analysis results showed that iCEB was positively correlated with Hcy and SYNTAXⅡ score in elderly patients with coronary heart disease combined with hypertension(P<0.05).Multivariate linear regression analysis showed that diastolic blood pressure[β=0.127,95%CI(0.038-0.216)],systolic blood pressure[β=0.085,95%CI(0.022-0.148)],high density lipoprotein cholesterol(HDL-C)[β=-1.872,95%CI(-3.237--0.507)],FBG[β=0.463,95%CI(0.155-0.771)],HbA1c[β=0.917,95%CI(0.356-1.478)],iCEB[β=3.217,95%CI(2.268-4.166)],SYNTAXⅡscore
