摘　　要：目的分析肺动脉高压(PAH)患者白介素(IL)-32、CC趋化因子配体28(CCL28)、前蛋白转化酶枯草溶菌素9(PCSK9)、肺循环血量(Qp)/体循环血量(Qs)表达情况、临床意义及其之间的关系。方法回顾性选取2023年10月—2024年10月内蒙古自治区人民医院心内科收治的PAH患者110例作为研究组,根据平均肺动脉压(mPAP)将其分为轻中度亚组(mPAP为25~40 mmHg)67例与重度亚组(mPAP>40 mmHg)43例。另选取同期于本院接受肺/体循环血量指标检测的非PAH患者108例作为对照组。比较对照组与研究组、轻中度亚组与重度亚组IL-32、CCL28、PCSK9水平及Qp、Qs、Qp/Qs。采用Pearson相关分析探讨PAH患者IL-32、CCL28、PCSK9水平及Qp、Qs、Qp/Qs与mPAP的相关性及IL-32、CCL28、PCSK9水平与Qp、Qs、Qp/Qs的相关性。结果研究组IL-32、CCL28、PCSK9水平及Qp、Qp/Qs高于对照组,Qs低于对照组(P<0.05)。重度亚组IL-32、CCL28、PCSK9水平及Qp、Qp/Qs高于轻中度亚组,Qs低于轻中度亚组(P<0.05)。Pearson相关分析结果显示,PAH患者IL-32、CCL28、PCSK9水平及Qp、Qp/Qs与mPAP呈正相关,Qs与mPAP呈负相关(P<0.05);PAH患者IL-32、CCL28、PCSK9水平均与Qp、Qp/Qs呈正相关,与Qs呈负相关(P<0.05)。结论PAH患者IL-32、CCL28、PCSK9水平呈高表达,Qp/Qs增大,其均与PAH病情严重程度相关,且IL-32、CCL28、PCSK9水平均与Qp/Qs呈正相关。Objective The analyse the expression and clinical significance of interleukin(IL)-32,CC chemokine ligand 28(CCL28),proprotein convertase subtilisin kexin 9(PCSK9),pulmonary circulation blood volume(Qp)/systemic circulation volume(Qs)in patients with pulmonary arterial hypertension(PAH)and their relationships.Methods A total of 110 patients with PAH who were admitted to the Department of Cardiology,Inner Mongolia Autonomous Region People's Hospital from October 2023 to October 2024 were retrospectively selected as research group,and they were divided into mild-to-moderate subgroup of 67 cases[mean pulmonary artery pressure(mPAP)was 25-40 mmHg]and severe subgroup of 43 cases(mPAP>40 mmHg)according to mPAP.Another 108 patients who tested pulmonary/systemic circulation blood flow indicators in the same period in this hospital were selected as control group.The IL-32,CCL28 and PCSK9 levels and Qp,Qs,Qp/Qs were compared between control group and study group,as well as mild-to-moderate subgroup and severe subgroup.Pearson correlation analysis was used to explore the correlation between IL-32,CCL28 and PCSK9 levels and Qp,Qs,Qp/Qs and mPAP in patients with PAH and the correlation between IL-32,CCL28 and PCSK9 levels and Qp,Qs,Qp/Qs.Results The IL-32,CCL28 and PCSK9 levels and Qp,Qp/Qs in the study group were higher than those in control group,while Qs was lower than that in control group(P<0.05).The IL-32,CCL28 and PCSK9 levels and Qp,Qp/Qs in severe subgroup were higher than those in mild-to-moderate subgroup,while Qs was lower than that in mild-to-moderate subgroup(P<0.05).The results of Pearson correlation analysis showed that,IL-32,CCL28 and PCSK9 levels and Qp,Qp/Qs were positively correlated with mPAP,while Qs was negatively correlated with mPAP in PAH patients(P<0.05);the IL-32,CCL28,and PCSK9 levels were positively correlated with Qp and Qp/Qs,and negatively correlated with Qs in PAH patients(P<0.05).Conclusion The levels of IL-32,CCL28 and PCSK9 in PAH patients are elevated,and Qp/Qs is increased.All of them are
