摘　　要：目的 分析血清CXC趋化因子配体8(CXCL8)、分泌型卷曲相关蛋白(sFRP)5水平与心房颤动合并心力衰竭患者心功能的关系及其在临床诊断中的意义。方法 选取2022年1月至2023年10月唐山中心医院收治的106例心房颤动患者作为研究对象,根据是否合并心力衰竭分为衰竭组(36例)和未衰竭组(70例)。检测患者左心室射血分数(LVEF)、左心房内径(LAD)及血清CXCL8、sFRP5水平。采用Pearson相关分析衰竭组血清CXCL8、sFRP5水平与LVEF、LAD的相关性。采用Spearman相关分析衰竭组患者血清CXCL8、sFRP5水平与纽约心脏病协会(NYHA)心功能分级的相关性。采用多因素Logistic回归分析心房颤动合并心力衰竭的影响因素。绘制受试者工作特征(ROC)曲线分析血清CXCL8、sFRP5对心房颤动合并心力衰竭的诊断价值。结果 衰竭组的LVEF及血清sFRP5水平低于未衰竭组,LAD及血清CXCL8水平高于未衰竭组,差异均有统计学意义(P<0.05)。衰竭组不同NYHA心功能分级患者血清CXCL8水平比较结果显示,Ⅰ~Ⅱ级<Ⅲ级<Ⅳ级,且任意两两比较,差异均有统计学意义(P<0.05)。衰竭组不同NYHA心功能分级患者血清sFRP5水平比较结果显示,Ⅰ~Ⅱ级>Ⅲ级>Ⅳ级,且任意两两比较,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,衰竭组患者血清CXCL8水平与LVEF呈负相关(r=-0.307,P=0.001),与LAD呈正相关(r=0.445,P<0.001),衰竭组患者血清sFRP5水平与LVEF呈正相关(r=0.266,P=0.006),与LAD呈负相关(r=-0.316,P<0.001)。Spearman相关分析结果显示,衰竭组患者NYHA心功能分级与血清CXCL8水平呈正相关(rs=0.328,P<0.001),与血清sFRP5水平呈负相关(rs=-0.367,P<0.001)。多因素Logistic回归分析结果显示,血清CXCL8水平升高,sFRP5水平降低是心房颤动患者合并心力衰竭的危险因素(P<0.05)。ROC曲线分析结果显示,血清CXCL8、sFRP5联合诊断心房颤动合并心力衰竭的曲线下面积(AUC)为0.907,大于CXCL8、sFRP5单�Objective To analyze the relationship between serum levels of CXC chemokine ligand 8(CXCL8)and secreted frizzled related protein 5(sFRP5)with cardiac function in patients with atrial fibrillation complicated with heart failure and their significance in the clinical diagnosis.Methods A total of 106 patients with atrial fibrillation admitted to Tangshan Central Hospital from January 2022 to October 2023 were selected as the study objects and divided into failure group(36 cases)and non-failure group(70 cases)according to whether they were complicated with heart failure.Left ventricular ejection fraction(LVEF),left atrial diameter(LAD)and serum levels of CXCL8 and sFRP5 in patients were detected.Pearson correlation analysis was used to analyze the correlation between serum CXCL8,sFRP5 levels and LVEF,LAD in the failure group.Spearman correlation analysis was used to analyze the correlation between serum CXCL8 and sFRP5 levels and New York heart association(NYHA)heart function grades classification in the failure group.Multivariate Logistic regression analysis was used to analyze the influencing factors of atrial fibrillation complicated with heart failure.Receiver operating characteristic(ROC)curves were plotted to analyze the diagnostic value of serum CXCL8 and sFRP5 for atrial fibrillation complicated with heart failure.Results The LVEF and sFRP5 in failure group were lower than those in non-failure group,the LAD and serum CXCL8 level in failure group were higher than those in non-failure group,and the differences were statistically significant(P<0.05).The comparison of serum CXCL8 levels in patients with different NYHA heart function grades in the failure group showed thatⅠ-Ⅱgrade<Ⅲgrade<Ⅳgrade,and any pairwise comparison showed statistically significant differences(P<0.05).The comparison of serum sFRP5 levels among patients with different NYHA heart function grades in the failure group showed thatⅠ-Ⅱgrade>Ⅲgrade>Ⅳgrade,and any pairwise comparison showed statistically significant differences(P<0.05
