摘  要:目的 探讨血清脂氧素A4(LXA4)、组蛋白去乙酰化酶6(HDAC6)与慢性阻塞性肺疾病急性加重期(AECOPD)患者肺功能及预后的关系。方法 选取2021年5月至2024年6月在邯郸二八五医院就诊的235例AECOPD患者作为AECOPD组,另选取同期在邯郸二八五医院就诊的181例稳定期慢性阻塞性肺疾病(COPD)患者作为SCOPD组,以及在邯郸二八五医院体检的268例的健康志愿者作为对照组。根据AECOPD患者治疗后的预后情况分为预后良好组和预后不良组。采用酶联免疫吸附试验检测血清LXA4、HDAC6水平,肺功能检测仪检测呼气峰值流量(PEF)、用力肺活量(FVC)和第1秒用力呼气容积(FEV1),并计算FEV1/FVC比值。采用Pearson相关分析预后不良组血清LXA4、HDAC6水平与肺功能指标、病程、COPD评估测试(CAT)评分的相关性。采用多因素Logistic回归分析AECOPD患者预后不良的影响因素。绘制受试者工作特征(ROC)曲线分析血清LXA4、HDAC6对AECOPD患者预后不良的预测价值。结果 AECOPD组病程长于SCOPD组,CAT评分及血清HDAC6水平高于SCOPD组,而FEV1、FVC、FEV1/FVC比值、PEF及血清LXA4水平低于SCOPD组,差异均有统计学意义(P<0.05)。对照组血清LXA4水平高于SCOPD组和AECOPD组,血清HDAC6水平均低于SCOPD组和AECOPD组,差异均有统计学意义(P<0.05)。173例患者纳入预后良好组,62例患者纳入预后不良组。预后不良组FEV1、FVC、FEV1/FVC比值、PEF及血清LXA4水平均低于预后良好组,而病程长于预后良好组,CAT评分及血清HDAC6水平均高于预后良好组,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,预后不良组血清LXA4水平与病程、CAT评分呈负相关(P<0.05),与FEV1、FVC、FEV1/FVC比值、PEF呈正相关(P<0.05)。预后不良组血清HDAC6水平与病程、CAT评分呈正相关(P<0.05),与FEV1、FVC、FEV1/FVC比值、PEF均呈负相关(P<0.05)。多因素Logistic回归分析结果显示,CAT评分、血清HDAC6水平升高及血�Objective To investigate the relationship between serum lipoxin A4(LXA4),histone deacetylase 6(HDAC6)and lung function and prognosis in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 235 AECOPD patients in Handan 285th Hospital from May 2021 to June 2024 were selected as the AECOPD group,and 181 stable chronic obstructive pulmonary disease(COPD)patients in Handan 285th Hospital during the same period were selected as the SCOPD group and 268 healthy volunteers who underwent physical examination in Handan 285th Hospital were selected as the control group.According to the prognosis of AECOPD patients after treatment,they were divided into good prognosis group and poor prognosis group.Serum LXA4 and HDAC6 levels were detected by enzyme-linked immunosorbent assay.Peak expiratory flow(PEF),forced vital capacity(FVC)and forced expiratory volume in one second(FEV 1)were detected by pulmonary function detector,and FEV 1/FVC ratio was calculated.Pearson correlation analysis was used to analyze the correlation between serum LXA4,HDAC6 levels and pulmonary function indexes,disease duration,COPD assessment test(CAT)score in the poor prognosis group.Multivariate Logistic regression was used to analyze the influencing factors of poor prognosis in AECOPD patients.The receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of serum LXA4 and HDAC6 for poor prognosis in AECOPD patients.Results The course of disease in AECOPD group was longer than that in SCOPD group,CAT score and serum HDAC6 level were higher than those in SCOPD group,while FEV 1,FVC,FEV 1/FVC ratio,PEF and serum LXA4 level were lower than those in SCOPD group,the differences were statistically significant(P<0.05).The control group had a significantly higher serum LXA4 level and a significantly lower serum HDAC6 level than those in the SCOPD and AECOPD groups,and the differences were statistically significant(P<0.05).A total of 173 patients were included in the good prognosis group
