摘 要:目的分析血清烟酰胺腺嘌呤二核苷酸磷酸氧化酶4(NOX-4)、信号转导和转录激活子3(STAT3)在肺炎新生儿发生细菌性感染的早期诊断及治疗评价中的作用。方法选取2021年8月至2023年8月在该院就诊的150例新生儿肺炎患儿作为研究对象,根据患儿是否发生细菌感染将其分为细菌感染性肺炎组(86例)和非细菌感染性肺炎组(64例),并统计7 d抗感染治疗患儿的疗效情况。采用酶联免疫吸附试验检测血清NOX-4、STAT3水平。采用Pearson相关分析细菌感染性肺炎新生儿血清NOX-4水平与STAT3水平的相关性。采用多因素Logistic回归分析肺炎新生儿发生细菌感染的影响因素。绘制受试者工作特征(ROC)曲线分析血清NOX-4、STAT3对新生儿细菌感染性肺炎的诊断价值。结果细菌感染性肺炎组血清C反应蛋白(CRP)、NOX-4、STAT3水平及心率均显著高于细菌感染性肺炎组,差异均统计学意义(P<0.05)。细菌感染性肺炎疗效不良新生儿血清NOX-4、STAT3水平均高于非细菌感染性肺炎疗效良好新生儿,非细菌感染性肺炎疗效不良新生儿血清NOX-4、STAT3水平均高于非细菌感染性肺炎疗效良好新生儿,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,细菌感染性肺炎新生儿血清NOX-4水平与STAT3水平呈正相关(r=0.421,P<0.05)。多因素Logistic回归分析结果显示,心率、CRP及血清NOX-4、STAT3水平升高是肺炎新生儿发生细菌性感染的危险因素(P<0.05)。ROC曲线分析结果显示,血清NOX-4、STAT3联合诊断新生儿细菌感染性肺炎的曲线下面积(AUC)为0.929,大于血清NOX-4、STAT3单独诊断的AUC(Z=2.083、3.846,P<0.05)。结论血清NOX-4、STAT3高表达均会造成肺炎新生儿发生细菌性感染的风险升高,检测血清NOX-4、STAT3水平在新生儿肺炎早期诊断和治疗中有积极意义。Objective To investigate the role of nicotinamide adenine dinucleotide phosphate oxidase 4(NOX-4)and signal transducer and activator of transcription 3(STAT3)in the early diagnosis and treatment evaluation of bacterial infection in neonates with pneumonia.Methods A total of 150 children with neonatal pneumonia who were treated in this hospital from August 2021 to August 2023 were selected as the study objects.According to whether the children had bacterial infection,they were divided into bacterial pneumonia group(86 cases)and non-bacterial pneumonia group(64 cases),and the efficacy of anti-infection treatment for 7 days was analyzed.Serum NOX-4 and STAT3 levels were detected by enzyme-linked immunosorbent assay.Pearson method was applied to analyze the correlation between serum NOX-4 and STAT3 levels of bacterical infection in neonates with pneumonia.Multivariate Logistic regression was applied to analyze the influencing factors of bacterial infection in neonates with pneumonia.Receiver operating characteristic(ROC)curve was applied to analyze the diagnostic value of serum NOX-4 and STAT3 levels for neonatal with bacterial pneumonia.Results The levels of serum C-reactive protein(CRP),NOX-4,STAT3 and heart rate in bacterial pneumonia group were significantly higher than those in non-bacterial pneumonia group,and the differences were statistically significant(P<0.05).Serum NOX-4 and STAT3 levels in neonates with poor efficacy of bacterial pneumonia were higher than those in neonates with good efficacy of bacterial pneumonia,and serum NOX-4 and STAT3 levels in neonates with poor efficacy of non-bacterial pneumonia were higher than those in neonates with good efficacy of non-bacterial pneumonia,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that there was a positive correlation between serum NOX-4 level and STAT3 level of bacterial infection in neonates with pneumonia(r=0.421,P<0.05).Multivariate Logistic regression analysis showed that increased heart rate,CRP,serum NOX-4
