家庭医生签约服务对社区老年慢性病患者就医行为及医疗费用的影响分析  

Analysis of the Impact of Family Doctor Contract Service on the Medical Behavior and Medical Expenses of Elderly Chronic Disease Patients in the Community

摘　　要：目的探讨家庭医生签约服务对社区老年慢性病患者就医行为模式转变和医疗费用的影响,为推进分级诊疗制度实施提供参考依据。方法选取某社区卫生服务中心2023年1—12月接受家庭医生签约服务的老年慢性病患者120例作为观察组,选取同期未签约的老年慢性病患者120例作为对照组。比较两组患者在基层就诊率、慢性病规范管理率、年度医疗总费用、基层医疗费用占比、合理用药率、患者满意度等指标的差异。结果观察组患者基层就诊率、慢性病规范管理率、基层医疗费用占比、合理用药率和患者满意度均显著高于对照组,年度医疗总费用低于对照组,差异均有统计学意义(P<0.05)。结论家庭医生签约服务能够有效引导老年慢性病患者合理就医,提高基层医疗资源利用效率,降低医疗费用,提升医疗服务满意度,对推进分级诊疗制度实施具有积极意义。Objective To explore the impact of family doctor contract service on the transformation of medical behavior pattern and medical expenses of elderly chronic disease patients in the community,and to provide a reference for promoting the implementation of the hierarchical diagnosis and treatment system.Methods 120 elderly chronic disease patients who received family doctor contract service from January to December 2023 in a community health service center were selected as the observation group,and 120 elderly chronic disease patients who did not sign the contract during the same period were selected as the control group.The differences in primary care visit rate,chronic disease standardized management rate,annual total medical expenses,proportion of primary medical expenses,rational drug use rate,and patient satisfaction were compared between the two groups.Results The primary care visit rate,chronic disease standardized management rate,proportion of primary medical expenses,rational drug use rate,and patient satisfaction of the observation group were significantly higher than those of the control group,and the annual total medical expenses were lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Family doctor contract service can effectively guide elderly patients with chronic diseases to seek medical treatment rationally,improve the utilization efficiency of primary medical resources,reduce medical expenses,and improve medical service satisfaction,which has positive significance for promoting the implementation of the hierarchical diagnosis and treatment system.
