摘　　要：目的 探讨心脏再同步化治疗(cardiac resynchronization therapy,CRT)的慢性心力衰竭患者心脏超声参数与左心室阈值的相关性。方法 回顾性分析南京大学医学院附属盐城第一医院心血管内科2015年1月—2023年10月收治的14例行CRT治疗的慢性心力衰竭患者临床及超声资料,所有患者因心脏扩大都伴有不同程度的二尖瓣关闭不全,分析各临床指标与左心室阈值的相关性。结果 Pearson相关性分析显示,左心室阻抗、二尖瓣环舒张早期运动速度峰值(early diastolic peak velocity of mitral valve annulus,E')与左心室阈值均呈正相关(r=0.625,P=0.017;r=0.667,P=0.009)。结论 伴二尖瓣关闭不全的CRT植入患者,左心室阻抗、二尖瓣环E'与左心室阈值明显相关。拟行CRT起搏的患者常规检测二尖瓣环E'能够为左心室高起搏阈值患者的识别提供一些线索。Objective To investigate the correlation between cardiac ultrasound parameters and left ventricular threshold in patients with chronic heart failure after cardiac resynchronization therapy(CRT).Methods The clinical and ultrasound data of 14 patients with chronic heart failure treated with CRT from January 2015 to October 2023 in the department of cardiovascular medicine of Yancheng First Hospital Affiliated of Nanjing University Medical College were retrospectively analyzed.All patients were accompanied by mitral valve insufficiency due to cardiac enlargement.The correlation between clinical indicators and left ventricular threshold was analyzed.Results Pearson correlation analysis showed that left ventricular impedance,early diastolic peak velocity of mitral valve annulus(E')were positively correlated with left ventricular threshold(r=0.625,P=0.017;r=0.667,P=0.009).Conclusion In patients with CRT implantation with mitral valve insufficiency, left ventricular impedance, mitral annulus E' and left Ventricular threshold is obviously related. Routine detection of mitral ring E' in patients with CRT pacing can provide some clues for the identification of patients with high left ventricular pacing threshold.
