独活寄生汤联合针灸治疗腰椎间盘突出的临床疗效及对其中医症状的影响

摘　　要：目的 研究独活寄生汤联合针灸治疗腰椎间盘突出(LDH)的临床疗效及对其中医症状的影响。方法 选取2021年7月-2023年7月峡江县中医院收治的68例LDH患者,经抽签法分为对照组(34例)与观察组(34例),对照组给予独活寄生汤治疗,观察组在其基础上联合针灸治疗,比较两组临床疗效、中医症状积分(腰腿疼痛、下肢麻木、筋骨痿软)、腰椎功能[腰椎功能评分表(JOA)、Oswestry功能障碍指数问卷表(ODI)]、腰椎活动度(后伸、前屈)。结果 观察组临床改善率高于对照组(P<0.05);两组治疗后中医症状积分(腰腿疼痛、下肢麻木、筋骨痿软)低于治疗前,且观察组低于对照组(P<0.05)。两组治疗后JOA评分高于治疗前,ODI评分低于治疗前,且观察组JOA评分高于对照组,ODI评分低于对照组(P<0.05)。两组治疗后腰椎活动度(后伸、前屈)大于治疗前,且观察组大于对照组(P<0.05)。结论 独活寄生汤联合针灸可提高LDH患者临床疗效,减轻其中医症状,改善腰椎功能,恢复患者的腰椎活动度。Objective To study the clinical efficacy of Duhuo Jisheng decoction combined with acupuncture and moxibustion in the treatment of lumbar disc herniation(LDH) and its effect on TCM symptoms.Methods A total of 68 patients with LDH admitted to Xiajiang County Hospital of Traditional Chinese Medicine from July 2021 to July 2023 were selected and divided into control group(34 patients) and observation group(34patients) by drawing lots.The control group was treated with Duhuo Jisheng decoction,and the observation group was treated with acupuncture and moxibustion on the basis of the control group.The clinical efficacy,TCM symptom scores(low back and leg pain,numbness of lower limbs,tendon and bone weakness),lumbar function [lumbar function score(JOA),Oswestry dysfunction index questionnaire(ODI)],and lumbar range of motion(extension and flexion) were compared between the two groups.Results The clinical improvement rate of the observation group was higher than that of the control group(P<0.05).After treatment,the scores of TCM symptoms(lumbocrural pain,numbness of lower limbs,flaccidity of muscles and bones) in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group( P<0.05).The JOA score of the two groups after treatment was higher than that before treatment,the ODI score was lower than that before treatment,while the JOA score of the observation group was higher than that of the control group,and the ODI score was lower than that of the control group( P<0.05).The lumbar range of motion(posterior extension and flexion) of the two groups after treatment was greater than that before treatment,and that of the observation group was greater than that of the control group(P<0.05).Conclusion Duhuo Jisheng decoction combined with acupuncture can improve the clinical efficacy of LDH patients,reduce their TCM symptoms,improve lumbar function,and restore lumbar mobility.
