微生态免疫营养剂联合血液净化治疗脓毒症的临床疗效

摘　　要：目的:分析脓毒症采用微生态免疫营养剂(MIN)联合血液净化治疗所发挥的作用。方法:选取2021年1月—2022年12月河南大学附属南阳市第一人民医院收治的66例脓毒症患者作为研究对象,按随机数表法分为对照组和观察组,每组各33例。在常规治疗的基础上,两组患者分别采取不同治疗,观察疗效。结果:治疗后,观察组患者序贯器官衰竭评估(SOFA)评分、急性生理与慢性健康状况评估(APACHEⅡ)明显较低,观察组患者C反应蛋白(CRP)水平、降钙素原(PCT)水平明显较低,观察组患者IgG、IgM、IgA水平明显较高,差异均有统计学意义(t=7.995、8.114、6.602、15.530、2.034、7.269、9.192,P<0.05);治疗7 d、治疗14 d,观察组患者的双歧杆菌、乳杆菌数量明显较多,葡萄球菌数量明显较少,差异均有统计学意义(t=21.468、18.709、2.961、5.599、9.229、6.645,P<0.05);治疗后,观察组总有效率较高,差异有统计学意义(χ^(2)=7.543,P<0.05)。结论:MIN联合血液净化治疗脓毒症,可有效改善炎症指标,增强患者免疫功能,改善患者肠道菌群,治疗效果良好。Objective:To analyze the effect of using microecological immunonutrients(MIN)combined with blood purification therapy in the treatment of sepsis.Methods:66 patients with sepsis in the hospital from January 2021 to December 2022 were selected,and they were divided into control group and observation group,with 33 patients in each group.On the basis of conventional treatment,the two groups received different treatments and the therapeutic effects were observed.Results:After treatment,SOFA and APACHEⅡscores were significantly lower in the observation group.C-reactive protein(CRP)and procalcitonin(PCT)levels were significantly lower in the observation group,and IgG,IgM and IgA levels were significantly higher in the observation group.The differences were statistically significant(t=7.9952,8.1141,6.6021,15.5302,2.0341,7.2691,9.1924;P<0.05).After 7 and 14 days of treatment,the number of bifidobacterium and Lactobacillus in observation group was significantly higher,while the number of staphylococcus was significantly lower,with statistically significant difference(t=21.4682,18.7092,2.9612,5.5991,9.2291,6.6452;P<0.05).After treatment,the curative effect of the observation group was better,and the difference was statistically significant(χ^(2)=7.5429,P<0.05).Conclusion:The combination of MIN and blood purification in the treatment of sepsis can effectively improve inflammatory indicators,enhance patient immune function,improve patient gut microbiota,and have good therapeutic effect.
