糖尿病早期肾损伤患者尿微量白蛋白肌酐比值及血清β2-微球蛋白、糖化血红蛋白检测的临床意义

摘　　要：目的:探究糖尿病(DM)早期肾损伤患者尿微量白蛋白肌酐比值(MALB/UCREA)及血清β2-微球蛋白(β2-MG)、糖化血红蛋白(HbA1c)检测的临床意义。方法:选取2022年1月—2023年12月郑州市第二人民医院收治的108例DM患者的临床资料进行回顾性分析,据早期糖尿病肾病(DKD)的诊断标准将患者分为早期DKD组(60例)和单纯DM组(48例),比较两组患者MALB/UCREA、血清β2-MG、HbA1c水平的差异,并应用受试者工作特征曲线(ROC)分析和ROC下面积(AUC)判断上述指标检测诊断早期DKD的价值。结果:早期DKD组患者MALB/UCREA、β2-MG、HbA1c水平高于单纯DM组,差异均有统计学意义(t=92.851、5.848、6.664,P<0.05);MALB/UCREA、β2-MG、HbA1c联合检测的AUC分别为1.000、0.796、0.823,对早期DKD的诊断均具有一定的价值,差异有统计学意义(P<0.05),且MALB/UCREA检测的灵敏度、特异度、准确率均高于β2-MG、HbA1c检测。结论:早期DKD患者MALB/UCREA、β2-MG、HbA1c均异常升高,对于DKD的诊断具有较高的灵敏度、特异度和准确率,且MALB/UCREA检测的诊断效能更高,可为早期疾病诊治提供依据。Objective:To explore clinical detection significance of urine microalbumine to creatinine ratio(MALB/UCREA),serumβ2-microglobulin(β2-MG)and hemoglobin A1c(HbA1c)in patients with early kidney injury of diabetes mellitus(DM).Methods:A retrospective analysis was performed on the clinical data of 108 patients with DM in the hospital from January 2022 to December 2023.According to the diagnostic criteria of early diabetic kidney disease(DKD),108 patients were divided into early DKD group(60 cases)and simple DM group(48 cases).The differences in MALB/UCREA,serumβ2-MG and HbA1c between the two groups were compared.The diagnostic value of the above indexes and combined detection for early DKD was determined by ROC curves analysis and area under ROC curve(AUC).Results:The levels of MALB/UCREA,β2-MG and HbA1c in early DKD group were higher than those in simple DM group(t=92.851,5.848,6.664;P<0.05).AUC values of MALB/UCREA,β2-MG,HbA1c and combined detection in the diagnosis of early DKD were 1.000,0.796 and 0.823,showing certain diagnostic value(P<0.05).The sensitivity,specificity and accuracy of combined detection were higher than those of single index.Conclusion:MALB/UCREA,β2-MG and HbA1c are abnormally increased in patients with early DKD,which all have high sensitivity,specificity and accuracy in the diagnosis of DKD.The diagnostic efficiency of combined detection is higher,which can provide basis for early diagnosis and treatment of the disease.
