MSCT灌注参数联合弥散加权成像在胃癌患者术前评估中的应用

摘　　要：目的:探讨多层螺旋CT(MSCT)灌注参数联合弥散加权成像在胃癌患者术前评估中的应用效果。方法:选取2021年9月—2023年9月河南科技大学第二附属医院收治的98例胃癌患者作为研究对象。患者术前均接受MSCT灌注成像检查、弥散加权成像检查,以术后病理检查结果为金标准,分析MSCT灌注参数联合弥散加权成像在胃癌患者术前评估中的应用价值。结果:临床分期为Ⅲ期的患者血流量、血容量、表面通透性数值均高于Ⅰ~Ⅱ期患者,表观弥散系数(ADC)值低于Ⅰ~Ⅱ期患者,差异均有统计学意义(t=4.440、4.153、3.472、4.302,P<0.05)。低分化患者表面通透性数值高于中高分化,ADC值低于中高分化,差异均有统计学意义(t=4.986、5.517,P<0.001)。MSCT灌注成像检查参数、弥散加权成像检查参数单独及联合评估胃癌患者术前临床分期的曲线下面积(AUC)分别为0.773、0.739、0.722、0.754、0.915,均具有一定评估价值,联合评估价值最高。MSCT灌注成像检查参数、弥散加权成像检查参数单独及联合评估胃癌患者术前分化程度的AUC分别为0.805、0.817、0.871,均具有一定评估价值,联合评估价值最高。结论:MSCT灌注参数联合弥散加权成像对胃癌患者术前临床分期及分化程度具有较高的评估价值。Objective:To explore the application effect of multi slice spiral CT(MSCT)perfusion parameters combined with diffusion-weighted imaging in preoperative evaluation of gastric cancer patients.Methods:98 gastric cancer patients admitted to the Second Affiliated Hospital of Henan University of Science and Technology from September 2021 to September 2023 were selected,all of whom were scheduled for surgical treatment and underwent MSCT perfusion imaging and diffusion-weighted imaging before surgery.The postoperative pathological examination results were used as the gold standard to analyze the application value of MSCT perfusion parameters combined with diffusion-weighted imaging in preoperative evaluation of gastric cancer patients.Results:The blood flow,blood volume,and surface permeability values of patients in clinical stage Ⅲ were higher than those in stages Ⅰ~Ⅱ,and the apparent diffusion coefficient(ADC)value was lower than that in stages Ⅰ~Ⅱ,with statistically significant difference(t=4.440,4.153,3.472,4.302;P<0.05).The surface permeability value of patients with low differentiation was higher than that of those with medium to high differentiation,while the ADC value was lower than that of those with medium to high differentiation,with statistically significant difference(t=4.986,5.517;P<0.001).The area under the curve(AUC)values for evaluating preoperative clinical staging of gastric cancer patients using MSCT perfusion imaging parameters and diffusion-weighted imaging parameters alone and in combination were 0.773,0.739,0.722,0.754,and 0.915,respectively,all of which had certain evaluation value,and the combined evaluation value was the highest.AUC values of MSCT perfusion imaging parameters and diffusion-weighted imaging parameters for evaluating the preoperative differentiation degree of gastric cancer patients alone and in combination were 0.805,0.817,and 0.871,respectively,all of which had certain evaluation value,and the joint evaluation value was the highest.Conclusion:The combination of MSCT
