摘  要:目的:探讨磁敏感加权成像(SWI)在早产儿脑损伤诊断中的应用价值。方法:回顾性分析2019年1月—2024年3月商丘市第三人民医院新生儿科收治的208例脑损伤早产儿[脑室周围-脑室内出血(PIVH)组]和100例无脑损伤的健康早产儿(健康对照组)临床资料。分析磁共振成像(MRI)常规序列T1WI、T2WI以及SWI的影像学表现。结果:T1WI成像呈高信号或等信号,T2WI成像呈低信号或高低混杂信号,SWI成像为低信号;SWI成像对PIVH检出率明显高于MRI常规序列,差异有统计学意义(χ^(2)=20.045,P<0.001);SWI成像评估PIVH病灶面积明显大于MRI常规序列,差异有统计学意义(Z=10.384,P<0.05);SWI成像对PIVH侧脑室、脉络丛、第四脑室、中脑导水管的病灶数目的检出率高于MRI常规序列,差异均有统计学意义(χ^(2)=8.041、10.085、15.938、9.608,P<0.05);PIVH组定量SWI成像的相位值大于健康对照组,差异有统计学意义(Z=4.854,P<0.05);定量SWI成像诊断PIVH的灵敏度为82.70%,特异度为75.00%。结论:相较MRI常规序列,SWI成像在脑损伤早产儿的诊断中更能发现微出血灶,对病灶的检出率及病灶数目和大小的评估更为准确,定量SWI成像也可作为补充方法。Objective:To explore the application value of susceptibility weighted imaging(SWI)in diagnosis of brain injury in premature infants.Methods:A retrospective study was conducted among 208 premature infants with brain injury(PIVH group)and 100 healthy premature infants(healthy control group)who were born during the period from January 2019 to March 2024.Imaging manifestations of conventional MRI sequences T1WI,T2WI and SWI were analyzed.Results:T1WI showed high signal or isointensity,T2WI showed low signal or high-low mixed signal,and SWI showed low signal.The detection rate of PIVH in SWI imaging was significantly higher than that in conventional MRI sequences,and the difference was statistically significant(χ^(2)=20.045,P<0.001).SWI imaging showed that the lesion area of PIVH was significantly larger than that of conventional MRI sequences,with statistically significant difference(Z=10.384,P<0.05).The detection rate of the number of lesions in PIVH lateral ventricle,choroid plexus,fourth ventricle and midbrain aquaquet by SWI imaging was higher than that in conventional MRI sequences,and the differences were statistically significant(χ^(2)=8.041,10.085,15.938,9.608;P<0.05).The phase value of quantitative SWI imaging in PIVH group was higher than that in healthy control group,and the difference was statistically significant(Z=4.854,P<0.05).The sensitivity and specificity of SWI imaging in the diagnosis of PIVH were 82.70%and 75.00%respectively.Conclusion:Compared with conventional MRI sequence,SWI can find microbleeds in diagnosis of premature infants with brain injury.Besides,it is more accurate in assessment of the number and size of lesions.Quantitative SWI can be used as a supplementary method.
