基于IMB模型的认知干预应用于特应性皮炎患儿家庭照护者的效果分析

摘　　要：目的:研究对儿童特应性皮炎(AD)患者的家庭照护者采取基于信息-动机-行为技巧(IMB)模型的认知干预后对患儿治疗效果、生活质量、家庭满意度的影响以及疾病认知水平与其相关性。方法:选取2023年1—6月中国医学科学院皮肤病医院过敏与风湿免疫科收治的80例AD患儿及其家庭照护者为研究对象,按入院顺序分为观察组(n=40)和对照组(n=40)。对照组采用常规健康教育,观察组在常规健康教育基础上行IMB模型的认知干预。比较两组患儿及照护者干预前后AD认知水平、AD评分指数(SCORAD)、照护者满意度、儿童皮肤病生活质量指数(CDLQI)情况。结果:干预后,观察组照护者的疾病认知水平高于对照组,差异有统计学意义(t=-8.770,P<0.05);观察组患儿疾病严重程度低于对照组,差异有统计学意义(t=-3.060,P<0.05);观察组照护者护理满意度高于对照组,差异有统计学意义(χ^(2)=6.565,P<0.05);观察组患儿CDLQI评分低于对照组,差异有统计学意义(t=-3.790,P<0.05)。结论:对AD患儿家庭照护者运用基于IMB模型的认知干预模式,可有效提升家庭照护者对疾病治疗和护理的认知水平,明显改善AD患儿的疾病症状,提高AD患儿家庭满意度和患儿生活质量。Objective:To study the effects of cognitive intervention based on information-motivation-behavioral skills(IMB)model on the treatment effect,quality of life,family satisfaction and the correlation of disease cognition of family caregivers of children with atopic dermatitis.Methods:A total of 80 children with atopic dermatitis and their family caregivers who received treatment in the Department of Allergy and Rheumatology,Dermatology Hospital,Chinese Academy of Medical Sciences from January 2023 to June 2023 were selected as the study objects.According to the admission order,the patients were divided into observation group(40 cases)and control group(40 cases).The conventional health education was applied to the control group,and on the basis of this,the cognitive intervention model based on the information-motivation-behavioral skills(IMB)model was applied to the observation group.Atopic dermatitis cognitive level evaluation scale,atopic dermatitis Score Index(SCORAD),parents’satisfaction evaluation scale,and Children’s Dermatology Life Quality Index(CDLQI)were compared.Results:After the intervention,the cognitive level of the caregivers in the observation group was higher than that in the control group,and the difference was statistically significant(t=-8.770,P<0.05).The severity of disease in observation group was lower than that in control group,and the difference was statistically significant(t=-3.060,P<0.05).The nursing satisfaction of caregivers in the observation group was higher than that in the control group,and the difference was statistically significant(χ^(2)=6.565,P<0.05).The CDLQI score in observation group was lower than that in control group,and the difference was statistically significant(t=-3.790,P<0.05).Conclusion:The appliction of cognitive intervention mode based on IMB model to family caregivers of children with atopic dermatitis can effectively improve their cognition level of disease treatment and nursing,significantly improve the disease symptoms of children with atopic dermatitis,and
