具身情绪视角下肺癌患者癌痛程度在自我感受负担与生命质量间的中介作用及关怀建议

摘　　要：目的探讨肺癌患者具身情绪视角下癌痛控制程度在自我感受负担与生活质量间的中介作用。方法对2023年6月-12月住院的276例诊断为肺癌的患者,采用自我感受负担量表(SPBS)、简明疼痛评估量表(BPI)、生活质量评价量表-36(SF-36)对患者进行调查,分析肺癌患者癌痛控制程度在自我感受负担与生活质量间的中介作用。结果276例肺癌患者癌痛程度得分(3.20±2.02)分,自我感受负担得分(31.51±4.86)分,生活质量得分为(75.98±8.51)分。生活质量与癌痛程度及自我感觉负担呈负相关,自我感觉负担与癌痛程度呈正相关。中介效应模型显示,癌痛程度在自我感受负担与生活质量间起部分中介作用,效应值为-0.57,占总效应的54.81%。结论肺癌患者自我感觉负担水平有待降低,中重度的自我感觉负担会影响患者表达疼痛的意愿,从而导致癌痛控制程度不佳,生活质量降低。Objective To explore the mediating effect of cancer pain control degree on perceived burden and quality of life in patients with lung cancer from the perspective of embodied emotion.Methods A total of 276 patients diagnosed with lung cancer who were hospitalized from June to December 2023 were investigated by self-perceived Burden Scale(SPBS),Simplified Pain Assessment Scale(BPI),and Quality of Life Assessment Scale-36(SF-36)to analyze the mediating role of cancer pain control degree in patients with lung cancer between self-perceived burden and quality of life.Results The score of cancer pain degree of 276 patients with lung cancer was(3.20±2.02),the score of self-perceived burden was(31.51±4.86),and the score of life quality was(75.98±8.51).The quality of life was negatively correlated with the degree of cancer pain and self-perceived burden,while the self-perceived burden was positively correlated with the degree of cancer pain.The mediating effect model showed that the degree of cancer pain played a partial mediating role in the relationship between self-perceived burden and quality of life,and the effect value was-0.57,accounting for 54.81%of the total effect.Conclusion The level of self-perceived burden of lung cancer patients needs to be reduced.Moderate to severe self-perceived burden will affect patients′willingness to express pain,resulting in poor control of cancer pain and reduced quality of life.
