摘  要:目的探讨过敏性哮喘(AA)患儿肠道菌群多样性与免疫球蛋白E(IgE)水平和CD4^(+)T细胞表达的相关性。方法选择2022年1月至2023年5月期间在我院门诊或住院治疗的AA患儿70例作为AA组(n=70),选择同期于我院体检的健康儿童70例作为对照组(n=70)。收集所有患儿临床资料并对其进行肠道菌群分析,并收集实验室检查结果(包括IgE水平和CD4^(+)CD25^(+)Foxp3^(+)T占比)相关资料。对比两组患儿上述资料之间的差异,并分析肠道菌群多样性与IgE水平和CD4^(+)CD25^(+)Foxp3^(+)T占比之间的关系。结果AA组患儿EOS、IgE显著高于对照组,而FEV 1/FVC、Treg占比显著低于对照组(P<0.05)。AA组颤杆菌克属(Oscillibacter)、多尔氏菌属(Dorea)、嗜胆菌属(Bilophila)、链球菌属(Streptococcus)、普雷沃氏菌(Prevotella)、韦荣氏球菌属(Veillonella)为优势菌群,而对照组双歧杆菌属(Bifidobacterium)、另枝菌属(Alistipes)、毛螺旋菌属(Parasutterella)、纺锤链杆属(Fusicatenibacter)、巨球型菌属(Megasphaera)、瘤胃球菌科(Ruminococcaceae)为优势菌群(LDA>4log10)。AA组Chao1指数、ACE指数及Shannon指数低于对照组,而Simpson指数高于对照组,差异具有统计学意义(P<0.05)。Oscillibacter、Dorea、Bilophila、Streptococcus、Prevotella、Veillonella与IgE表达量呈正相关,与Treg占比呈负相关;Bifidobacterium、Alistipes、Parasutterella、Fusicatenibacter、Megasphaera、Ruminococcaceae与IgE表达量呈负相关,与Treg占比呈正相关(P<0.05)。结论AA患儿肠道菌群多样性情况与IgE及Treg占比显著相关。Objective To investigate the relationship between intestinal flora diversity and immunoglobulin E(IgE)level and CD4^(+)T cell expression in children with allergic asthma(AA).Methods A total of 70 children with AA who received outpatient or inpatient treatment in our hospital from January 2022 to May 2023 were selected as the AA group(n=70)and 70 healthy children who underwent physical examination in our hospital during the same period were selected as the control group(n=70).Clinical data of all children were collected,intestinal flora analysis was performed,and laboratory results(including IgE level and CD4^(+)CD25^(+)Foxp3^(+)T proportion)were collected.The differences in the above data between the two groups were compared,and the relationship between intestinal flora diversity,IgE level,and CD4^(+)CD25^(+)Foxp3^(+)T proportion was analyzed.Results EOS and IgE in the AA group were significantly higher than those in the control group,while the ratio of FEV 1/FVC and Treg in the AA group was significantly lower than that in the control group(P<0.05).In group AA,Oscillibacter,Dorea,Bilophila,Streptococcus,Prevotella,and Veillonella were the dominant bacterial groups.In the control group,Bifidobacterium,Alistipes,Parasutterella Fusicatenibacter,Megasphaera,and Ruminococcaceae were the dominant bacterial groups(LDA>4log10).Chao1 index,ACE index,and Shannon index in the AA group were lower than those in the control group,while the Simpson index was higher than those in the control group,the difference was statistically significant(P<0.05).Oscillibacter,Dorea,Bilophila,Streptococcus,Prevotella,and Veillonella were positively correlated with IgE expression,and negatively correlated with Treg proportion.Bifidobacterium,Alistipes,Parasutterella,Fusicatenibacter,Megasphaera,and Ruminococcaceae were negatively correlated with IgE expression and positively correlated with Treg proportion(P<0.05).Conclusion The diversity of intestinal flora in children with AA was significantly correlated with the proportion of IgE and Treg.
