摘　　要：目的:探讨简化体外循环在全胸腔镜再次三尖瓣手术中应用的安全性。方法:回顾性收集2020年6月—2023年12月在我院行简化体外循环下全胸腔镜再次三尖瓣手术的16例患者的临床资料。所有患者均采用单根多侧孔股静脉引流加股动脉灌注的插管方式行简化体外循环下全胸腔镜手术。统计手术时间、体外循环时间和术后带呼吸机时间。结果:16例患者中,14例行三尖瓣置换术,2例行三尖瓣修复。手术时间(144.8±20.6)min,体外循环时间(76.4±11.7)min。无体外循环相关神经系统并发症。术后带呼吸机时间(17.3±3.8)h,ICU停留时间(24.4±5.4)h,术后平均住院时间(5.8±2.0)d。结论:简化体外循环应用于全胸腔镜再次三尖瓣手术,简化了手术操作,围术期结果满意,值得临床推广应用。Objective To explore the safety of simplified cardiopulmonary bypass in total thoracoscopic reoperation tricuspid valve surgery.Methods The clinical data of 16 patients who underwent total thoracoscopic reoperation valve surgery under simplified cardiopulmonary bypass in our hospital from June 2020 to December 2023 were retrospectively collected. All patients underwent total thoracoscopic surgery under simplified cardiopulmonary bypass using a single multi-side hole femoral venous drainage cannula and femoral artery perfusion. The operation time, extracorporeal circulation time, and postoperative ventilator time were counted.Results Among the 16 patients, 14 underwent tricuspid valve replacement, and 2 underwent tricuspid valve repair. The operation time was(144.8±20.6) min, and the extracorporeal circulation time was(76.4±11.7) min. There were no neurological complications related to cardiopulmonary bypass. The postoperative ventilator time was(17.3±3.8) h, the ICU stay time was(24.4±5.4) h, and the average postoperative hospital stay was(5.8±2.0) d.Conclusion Simplified cardiopulmonary bypass is used in total thoracoscopic reoperation tricuspid valve surgery, which simplifies the surgical operation and has satisfactory perioperative results, and is worthy of clinical promotion and application.
