基于华生人性照护理论的康复护理在艾滋病合并结核病患者中的应用

摘　　要：目的探讨基于华生人性照护理论的康复护理在艾滋病合并结核病患者中的应用效果。方法选取2022年4月至2024年3月在郑州市第六人民医院诊治的260例艾滋病合并结核病患者作为研究对象,按随机数表法分为对照组和研究组各130例。对照组患者接受常规康复护理,研究组患者接受基于华生人性照护理论的康复护理,两组患者均护理至出院,出院后随访3个月。比较两组患者随访3个月后的护理依从性、护理前后自我效能及并发症发生率。结果随访3个月后,研究组患者护理依从性为91.54%,高于对照组的75.38%,差异有统计学意义(P<0.05);护理前,两组患者的自我减压、自我决策、正性态度评分比较差异无统计学意义(P>0.05),随访3个月后,两组患者的自我减压、自我决策、正性态度评分均升高,且研究组患者的自我减压、自我决策、正性态度评分分别为(37.43±5.31)分、(17.61±3.40)分、(54.08±6.02)分,高于对照组的(33.07±5.42)分、(13.34±4.21)分、(47.24±5.88)分,差异均有统计学意义(P<0.05);随访期间,研究组患者的并发症发生率为8.46%,明显低于对照组的26.15%,差异有统计学意义(P<0.05)。结论基于华生人性照护理论的康复护理在艾滋病合并结核病患者中的应用效果显著,可提高患者护理依从性及自我效能,同时能降低并发症发生率。Objective To explore the application effect of rehabilitation nursing based on Watson's theory of human caring in AIDS patients with tuberculosis.Methods A total of 260 patients with AIDS combined with tuberculosis,who were diagnosed and treated in Zhengzhou Sixth People's Hospital from April 2022 to March 2024 were selected.According to the random number table method,they were divided into a control group and a study group,with 130 cases in each group.The control group received routine rehabilitation nursing,while the study group received rehabilitation nursing based on Watson's theory of human caring.Both groups of patients were nursed until discharge and followed up for three months after discharge.Nursing compliance,self-efficacy,and complication rate were compared between the two groups after 3 months of follow-up.Results Three months after the follow-up,the nursing compliance rate of patients in the study group was 91.54%,which was significantly higher than 75.38%of the control group(P<0.05).Before nursing,there was no statistically significant difference in the scores of self-stress reduction,self-decision-making,and positive attitude between the two groups(P>0.05).However,three months after the follow-up,these scores increased in both groups,and the scores in the study group for self-stress reduction,self-decision-making,and positive attitude were(37.43±5.31)points,(17.61±3.40)points,and(54.08±6.02)points,respectively,which were higher than corresponding(33.07±5.42)points,(13.34±4.21)points,and(47.24±5.88)points of the control group,and the differences were statistically significant(P<0.05).During the follow-up period,the incidence of complications in the study group was 8.46%,significantly lower than 26.15%in the control group(P<0.05).Conclusion Rehabilitation nursing based on Watson's theory of human caring has a significant application effect on AIDS patients with tuberculosis.It can improve nursing compliance and self-efficacy while reducing the incidence of complications.
