摘 要:目的探究三维超声定位胎儿脊髓圆锥位置的应用价值。方法选取2020年1月至2023年12月于吉安市儿童医院进行产检的161例孕妇作为研究对象,所有孕妇均行三维超声定位检查,计算图像获取率,分析胎儿脊髓圆锥位置分布情况,采用线性回归分析脊髓圆锥位置与孕龄的关系。结果孕20~22周正常胎儿脊髓圆椎末端多位于L_(3)至L_(4),孕23~24周正常胎儿脊髓圆椎末端多位于L_(2~3)椎体间至L_(3~4)椎体间,孕25~27周正常胎儿脊髓圆椎末端多位于L_(2)至L_(3),孕28~30周正常胎儿脊髓圆椎末端多位于L_(2)及L_(2~3)椎体间;通过三维超声对脊髓圆锥定位显示,圆锥位置异常的分别有4例位于L_(4)、6例位于L_(4~5)椎体间、1例位于L 5、9例位于L_(5)~S_(5)。三维超声图像获取率为100.00%;线性回归分析结果显示,胎儿脊髓圆锥末端位置与孕龄呈正相关(R^(2)=0.812,P<0.001),线性回归方程为:Y=0.092 X+5.643。结论三维超声定位能获得胎儿脊髓圆锥的完整图像,直观地呈现胎儿椎体形态,脊髓圆锥位置定位具有较高的精准性,有助于尽早采取干预措施,值得临床推广应用。Objective To explore the application value of 3D ultrasound in locating the position of fetal spinal cord cone.Methods 161 pregnant women who underwent prenatal examinations at Ji'an Children's Hospital from January 2020 to December 2023 were selected as the research subjects,all pregnant women underwent 3D ultrasound localization examination,and the image acquisition rate was calculated,the distribution of fetal spinal cord cone position was analyzed,and linear regression was used to analyze the relationship between spinal cord cone position and gestational age.Results The rounded vertebral ends of the normal fetal spinal cord at 20 to 22 weeks of gestation were mostly located at L_(3) to L_(4),the rounded vertebral ends of the normal fetal spinal cord at 23 to 24 week of gestation were mostly located between the L_(2-3) vertebrae to L_(3-4) vertebrae,the rounded vertebral ends of the normal fetal spinal cord at 25 to 27 weeks of gestation were mostly located at L_(2) to L_(3),the rounded vertebral ends of the normal fetal spinal cord at 28 to 30 weeks of gestation were mostly located at L_(2) and L_(2-3) vertebrae;3D ultrasound localization of the spinal cord cones showed that the cones were abnormally located in 4 cases at L_(4),6 cases at L_(4-5) vertebrae,1 case at L 5 and 9 cases at L_(5)-S_(5),respectively.The acquisition rate of three-dimensional ultrasound images was 100.00%;linear regression analysis showed a positive correlation between the position of the terminal end of the fetal spinal cord cone and the week of gestation(R^(2)=0.812,P<0.001),and the linear regression equation was:Y=0.092 X+5.643.Conclusion 3D ultrasound localization can obtain complete images of the fetal spinal cord cone,visually presenting the morphology of the fetal vertebral body,the positioning of the spinal cord cone has high accuracy,which is helpful for early intervention measures and is worthy of clinical promotion and application.
