摘  要:目的探讨基于定量CT(quantitative computed tomography,QCT)的中老年男性腰椎骨密度(bone mineral density,BMD)、脂肪面积与冠状动脉钙化积分(calcium score,CS)的相关性。方法回顾性分析2018年4月至2019年6月于中山大学附属第五医院行胸部CT检查的280例中老年男性冠状动脉钙化患者的临床资料。利用QCT测量腰椎BMD值、内脏脂肪面积(visceral fat area,VFA)及皮下脂肪面积(subcutaneous fat area,SFA),应用后处理软件检测冠状动脉CS。比较不同冠状动脉钙化程度患者的临床资料,采用Pearson相关性分析CS与各指标的关系。结果不同冠状动脉钙化程度患者年龄、体重指数(body mass index,BMI)、BMD比较差异有统计学意义(P<0.05);冠状动脉重度钙化患者年龄大于轻度钙化和极少量钙化患者,BMD低于轻度钙化和极少量钙化患者,BMI低于中度钙化、轻度钙化和极少量钙化患者,中度钙化、轻度钙化患者年龄均大于极少量钙化患者,差异有统计学意义(P<0.05);不同冠状动脉钙化程度患者其余指标比较差异无统计学意义。冠状动脉CS与年龄呈正相关(r>0,P<0.05),与BMD呈负相关(r<0,P<0.05),而与VFA、SFA、VFA/SFA、BMI、血糖、总胆固醇、甘油三酯、低密度脂蛋白胆固醇、高密度脂蛋白胆固醇及同型半胱氨酸均无相关性。结论在中老年男性患者中,冠状动脉CS与腰椎BMD呈负相关,与VFA、SFA及VFA/SFA无明显相关。Objective To explore the correlation between bone mineral density(BMD),fat area and coronary artery calcification score(CS)in middle-aged and elderly male lumbar spine based on quantitative computed tomography(QCT).Methods The clinical data of 280 patients with coronary artery calcification in middle-aged and elderly men who underwent chest CT examination at The Fifth Affiliated Hospital,Sun Yat-sen University from April 2018 to June 2019 was retrospective analysed,lumbar BMD values,visceral fat area(VFA)and subcutaneous fat area(SFA)were measured using QCT,and post-processing software was applied to detect coronary CS.The clinical data was compared among the patients with different degrees of coronary artery calcification,Pearson correlation analysis was used to analyze the relationship between CS and each indicator.Results There were significant differences in age,body mass index(BMI)and BMD among the patients with different degrees of coronary artery calcification(P<0.05);the age in the severe calcification patients was greater than that in the mild calcification and the minimal calcification patients,the BMD was lower than that in the mild calcification and the minimal calcification patients,the BMI was lower than that in the moderate calcification,mild calcification and the minimal calcification patients,the age in the moderate and the mild calcification patients were greater than that in the minimal calcification patients,and the differences were statistically significant(P<0.05);there was no significant difference in the other indicators among the four groups.Coronary artery CS was positively correlated with age(r>0,P<0.05)and negatively correlated with BMD(r<0,P<0.05),and there were no significant correlation between the coronary artery CS and VFA,SFA,VFA/SFA,BMI,blood glucose,total cholesterol,triglycerides,low-density lipoprotein cholesterol,high-density lipoprotein cholesterol and homocysteine.Conclusion In middle-aged and elderly male patients,coronary artery CS was negatively correlated with lumbar B
