居家重度失能老人“互联网+护理服务”认知及意愿现状分析

摘　　要：目的调查居家重度失能老人对“互联网+护理服务”的认知及意愿现状并分析其影响因素,为推进“互联网+护理服务”持续深化发展提供参考。方法采用多阶段分层抽样法,选取云南省7个地级市共40个社区(村)115名居家重度失能老人作为研究对象,采用自行设计的一般资料调查表和国内学者开发的居家失能老年人“互联网+护理服务”认知意愿量表进行结构式问卷调查。结果75.65%的居家重度失能老人对“互联网+护理服务”不了解;65.22%的表示愿意使用“互联网+护理服务”;乡村户籍、高中或中专学历及护工照护的居家重度失能老人更愿意使用“互联网+护理服务”;费用医疗保险不报销、安全问题及应急措施、出现医疗纠纷难以判定责任是其“互联网+护理服务”意愿主要的消极影响因素。结论居家重度失能老人对“互联网+护理服务”认知程度较低但其意愿呈现积极发展趋势。建议政府制定政策,相关医疗机构及社区开展专项讲座与培训,建立失能老人互助平台以提高失能老人“互联网+护理服务”认知水平,推动“互联网+护理服务”健康发展。Objective To investigate the status of the cognition and willingness of Internet+nursing service for the elderly with severe disabilities at home and analyze influencing factors,so as to provide reference for the promotion of the continuous and deepening development of"Internet+nursing service".Methods Adopting multi-stage stratified sampling method,115 elderly with severe disabilities at home from 40 communities(villages)in 7 prefectural-level cities in Yunnan Province were selected as the research subj-ects,and a structured questionnaire survey was conducted by using the self-designed General Information Questionnaire and the"Internet+Nursing Service"Cognitive Willingness Scale developed by domestic scholars for elderly with severe disabilities at home.Results 75.65%of the elderly with severe disabilities at home did not know about"Internet+nursing service";65.22%of them were willing to use"Internet+nursing service";the elderly with severe disabilities at home with rural household registration,high school or middle school education and caregivers were more willing to use"Internet+nursing service"than those with rural household registration,high school or middle school education and carers."The main negative factors influencing their willingness to use"Internet+nursing service"are the lack of reimbursement by medical insurance,safety and first aid measures,and the difficulty of determining responsibility in case of medical disputes.Conclusion The level of awareness of"Internet+nursing service"among elderly with severe disabilities at home is low,but their willingness to use it shows a positive trend.It is suggested that the government should formulate policies,relevant medical institutions and communities should carry out special lectures and training,and a mutual help platform for the disabled elderly should be established to improve the level of awareness of"Internet+nursing service"and promote the healthy development of"Internet+nursing service".
