冠心病患者自我感受负担与二级预防用药依从性的相关性研究

摘　　要：目的本研究旨在了解冠心病患者自我感受负担对二级预防用药依从性的影响,为临床护理工作提供依据。方法采用横断面调查研究法,以2022年4—6月北京市某三甲医院心内科住院的冠心病患者为研究对象,通过一般资料调查表、Morisky用药依从性量表、自我感受负担量表进行调查。结果冠心病患者Morisky用药依从性量表平均得分为(5.65±1.69)分,40.6%的患者用药依从性较差,45.0%的中等,仅14.4%的用药依从性好。冠心病患者二级预防用药依从性与自我感受负担呈正相关关系,此外还受年龄、婚姻状态、服药数量、PCI治疗的影响。结论冠心病患者自我感受负担正向影响二级预防用药依从性,护士在临床工作中应注意定期评估冠心病患者的心理状态,密切观察其心理变化,同时要注重发挥家属在提高冠心病患者二级预防用药依从性中的积极作用。Objective To learn the impact of self-perceived burden and medication adherence for secondary prevention in patients with coronary atherosclerotic heart disease,and provide a basis for clinical nursing.Methods A cross-sectional survey was conducted on patients with coronary atherosclerotic heart disease admitted to the cardiology department of a Grade-A hospital in Beijing from April to June 2022.The study included a general information questionnaire conduct a survey with the Eighit-Item Morisky Medication Adherence and Self-perceived Burden Scale.Results The average score of the Eighit-Item Morisky Medication Adherence for patients with coronary atherosclerotic heart disease was(5.65±1.69)score.40.6%of patients had poor medication adherence,45.0%had moderate medication adherence,and only 14.4%had good medication adherence.There was a positive correlation between medication adherence for secondary prevention and self-perceived burden in patients with coronary atherosclerotic heart disease,and it was also influenced by age,marital status,medication quantity,and PCI treatment.Conclusion The self-perceived burden of patients with coronary atherosclerotic heart disease has a positive impact on their compliance with secondary preventive medication.Nurses should pay attention to regularly evaluating the psychological status of patients with coronary atherosclerotic heart disease and closely observing their psychological changes in clinical work.At the same time,they should also pay attention to the positive role of family members in improving the compliance of patients with coronary atherosclerotic heart disease with secondary prevention.
