摘　　要：目的探讨全身麻醉下实施腹膜后肿瘤切除术患者Century clot分析仪检测参数和常规凝血功能检测参数的相关性。方法回顾性选取2021年12月至2024年5月北京大学国际医院在全身麻醉下实施腹膜后肿瘤切除术的患者145例。记录患者入室时和手术结束时Century clot分析仪检测参数,即激活凝血时间(activated clotting time,ACT)、凝血速率(clot rate,CR)和血小板功能(platelet function,PF),术前和术后当日或第1天的常规凝血功能检测参数,即PT、APTT、凝血酶时间(thrombin time,TT)、FIB和PLT;采用Pearson相关性分析患者术前及术后Century clot分析仪检测参数与常规凝血功能检测参数的相关性及Century clot分析仪各指标之间的相关性。结果145例患者中,男50例、女95例,年龄24~78岁,平均(54.7±12.4)岁,身高平均(163.3±8.0)cm,体质量平均(63.6±13.2)kg,麻醉时间平均(331.4±89.6)min。Pearson相关性分析结果显示,术前及术后ACT与APTT呈正相关;CR与FIB和PLT呈正相关;PF与FIB和PLT呈正相关。术前及术后CR与PF呈正相关,与ACT呈负相关。结论在腹膜后肿瘤切除术患者中,Century clot分析仪检测参数与常规凝血功能检测参数具有显著相关性和一致性,能够对患者围术期出凝血功能做出较为快速、准确的检测,可用作目标导向性输血管理的辅助监测手段。Objective To explore the correlation between the detection parameters of the Century clot analyzer and the routine coagulation function test parameters in patients underwent retroperitoneal tumor resection under general anesthesia.Methods A total of 145 patients who underwent retroperitoneal tumor resection under general anesthesia in Peking University International Hospital from December 2021 to May 2024 were retrospectively selected.The detection parameters of the Century clot analyzer at the time of entering the operating room and at the end of the surgery were recorded,including activated clotting time(ACT),clot rate(CR),and platelet function(PF).The routine coagulation function test parameters before surgery and on the day of or the first day after surgery were recorded,including PT,APTT,thrombin time(TT),FIB and PLT.The correlation between the detection parameters of the Century clot analyzer and the routine coagulation function test parameters before and after surgery,and the correlation among the indicators of the Century clot analyzer were analyzed by Pearson correlation analysis.Results Among the 145 patients,there were 50 males and 95 females,aged from 24 to 78 years,with an average age of(54.7±12.4)years,The average height was(163.3±8.0)cm,the average weight was(63.6±13.2)kg,and the average anesthesia time was(331.4±89.6)min.The results of Pearson correlation analysis showed that before and after surgery,ACT was positively correlated with APTT.CR was positively correlated with FIB and PLT.PF was positively correlated with FIB and PLT.Before and after surgery,CR was positively correlated with PF and negatively correlated with ACT.Conclusions In patients underwent retroperitoneal tumor resection,there is a significant correlation and consistency between the detection parameters of the Century clot analyzer and the routine coagulation function test parameters.The Century clot analyzer can quickly and accurately detect the peri-operative coagulation function of patients and can be used as an auxiliary
