摘　　要：目的探讨泛免疫炎症值(PIV)与脓毒症并发急性呼吸窘迫综合征(ARDS)患者疾病转归的关系。方法选取132例脓毒症并发ARDS患者,根据氧合指数(OI)分为轻度组(n=47)、中度组(n=51)和重度组(n=34),检测并计算PIV。根据28天疾病转归分为死亡组(n=43)和生存组(n=89)。采用受试者操作特征曲线分析PIV对疾病转归的评估价值,并通过多因素Logistic回归分析其影响因素。结果死亡组机械通气时间≥3天比例、器官衰竭个数≥2个比例高于生存组;PIV水平轻度组<中度组<重度组,生存组<死亡组;OI水平重度组<中度组<轻度组,死亡组<生存组(P<0.05)。PIV与OI呈负相关(P<0.05)。多因素Logistic回归分析发现,机械通气时间≥3天、脏器衰竭个数≥2个、PIV高表达是疾病转归的影响因素(P<0.05);PIV对脓毒症并发ARDS患者疾病转归具有良好的评估价值(P<0.001)。结论高PIV值与脓毒症并发ARDS患者病情严重程度密切相关,可能是一种评估预后的新型血液学指标。Aim To investigate the correlation between pan-immune inflammatory value(PIV) and disease outcomes in patients with sepsis complicated by acute respiratory distress syndrome(ARDS).Methods A total of 132 patients with sepsis complicated by ARDS were selected and divided into mild group(n=47),moderate group(n=51) and severe group(n=34),according to oxygenation index(OI).PIV was detected and calculated.According to the 28-day disease outcomes,they were divided into death group(n=43) and survival group(n=89).The evaluation value of PIV on disease outcomes was analyzed by subject operation characteristic curve,and the influencing factors were analyzed by multivariate Logistic regression.Results The proportion of mechanical ventilation time ≥3 days and the number of organ failure ≥2 in the death group were higher than those in the survival group.PIV level was in an order of mild group<moderate group<severe group,and survival group<death group.OI level was in an order of severe group<moderate group<mild group,and death group<survival gropu(P<0.05).PIV was negatively correlated with OI,OI level(P<0.05).Multivariate Logistic regression analysis showed that mechanical ventilation duration ≥3 days,number of organ failure ≥2,high expression of PIV were the influencing factors of disease outcomes(P<0.05).PIV has a good value in evaluating the disease outcomes of patients with sepsis complicated by ARDS(P<0.001).Conclusion High PIV is closely related to the severity of ARDS patients with sepsis,and may be a new hematological index to evaluate the prognosis.
