摘 要:目的:探讨振幅整合脑电图(aEEG)联合新生儿行为神经测定(NBNA)评分在足月新生儿高胆红素血症脑损伤早期评估中的应用效果.方法:选取 2022 年 1 月至 2023 年 10 月本院收治的 42 例新生儿高胆红素血症患儿作为研究对象.患儿均接受aEEG监测、NBNA评分检测及MRI检查.记录MRI检查、aEEG监测及NBNA评分对患儿脑损伤检出率.以MRI检出结果为"金标准",评估aEEG监测、NBNA评分及联合检测结果与MRI检出结果的一致性.结果:经MRI检查显示,发生脑损伤患儿为 20 例,占 47.62%.aEEG监测、NBNA评分对患儿的脑损伤检出率分别为 45.24%(19/42)和 42.86%(18/42).经kappa检验显示,aEEG监测、NBNA评分及联合检测结果与MRI检出结果的一致性均较好,其中联合检测结果与MRI检出结果一致性最优.结论:aEEG监测、NBNA评分对足月新生儿高胆红素血症患儿脑损伤均有一定的评估价值,且两者联合评估价值更高.Objective:To explore the application effect of amplitude-integrated electroencephalography(aEEG)combined with neonatal behavioral neurological assessment(NBNA)in early assessment of hyperbilirubinemia induced brain injury in full-term newborns.Methods:A total of 42 children with neonatal hyperbilirubinemia admitted to our hospital from January 2022 to October 2023 were selected as the study subjects.All subjects received aEEG,NBNA scores and MRI.The detection rate of brain injury by MRI examination,aEEG monitoring and NBNA score were recorded.The results of MRI were used as the"gold standard"to evaluate the consistency between aEEG monitoring,NBNA score and combined detection results and MRI results.Results:MRI showed that 20 children had brain injury,accounting for 47.62%.The detection rate of brain injury detected by aEEC monitoring and NBNA score were 45.24%(19/42)and 42.86%(18/42),respectively.The kappa test showed that the aEEG monitoring,NBNA score and combined test results had good consistent with the MRI detection results,and the combined test results were the most consistent with the MRI detection results.Conclusion:The aEEG monitoring and NBNA score have certain evaluation value in the early assessment of hyperbilirubinemia induced brain injury in full-term newborns,and the combined evaluation value of the two is higher.
