Aspirex血栓抽吸术联合置管溶栓治疗对下肢深静脉血栓患者的影响

摘　　要：目的:探讨Aspirex血栓抽吸术联合置管溶栓治疗(CDT)对下肢深静脉血栓(DVT)患者临床疗效、凝血功能及预后复发的影响.方法:选取 2019 年 7 月至 2023 年 10 月本院收治的 88 例DVT患者为研究对象.按照治疗方案,将其分为观察组(n=47,予以Aspirex血栓抽吸术联合CDT治疗)和对照组(n=41,仅予以CDT治疗).比较两组患者临床疗效、复发率、凝血功能[凝血酶原时间(PT)、凝血活酶时间(TT)、纤维蛋白原浓度(FIB)、D-二聚体(D-D)水平]和并发症发生率.结果:观察组患者临床疗效总有效率高于对照组,观察组患者复发率低于对照组,差异均有统计学意义(P<0.05);治疗后,两组患者PT、TT时间均延长,D-D、FIB水平均降低,其中观察组PT、TT时间均长于对照组,D-D、FIB水平均低于对照组,差异均有统计学意义(P<0.05);观察组患者并发症发生率显著低于对照组,差异有统计学意义(P<0.05).结论:Aspirex血栓抽吸术联合CDT治疗DVT的临床疗效较好,具有术后复发率低、有效改善凝血功能的优点,且安全性较高.Objective:To explore the effect of Aspirex thrombus aspiration combined with catheter-directed thrombolysis(CDT)on clinical outcome,coagulation function and prognostic recurrence in patients with lower extremity deep vein thrombosis(DVT).Methods:A total of 88 patients with DVT admitted to our hospital from July 2019 to October 2023 were selected as the research objects.According to the treatment plan,the patients were divided into observation group(n=47,treated with Aspirex thrombus aspiration combined with CDT)and control group(n=41,treated with CDT only).The clinical efficacy,recurrence rate,coagulation function[prothrombin time(PT),thrombin time(TT),fibrinogen concentration(FIB),D-Dimer(D-D)],incidence of complications were compared between the two groups.Results:The total effective rate of observation group was significantly higher than that of control group,and the recurrence rate of observation group was lower than that of control group,with statistical significances(P<0.05).The PT and TT of the two groups were prolonged,and the D-D and FIB were decreased.The PT and TT of observation group were longer than those of control group,and the D-D and FIB were lower than those of control group,with statistically significant differences(P<0.05).The incidence of complications in observation group was significantly lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion:Aspirex thrombus aspiration combined with CDT has good clinical efficacy in the treatment of DVT,with the advantages of low postoperative recurrence rate,effective improvement of coagulation function,and acceptable safety.
