摘　　要：目的:探讨注射用尿促性腺激素(HMG)联合枸橼酸氯米芬对多囊卵巢综合征(PCOS)导致的不孕患者的临床疗效.方法:选取 2022 年 1 月至 2023 年 12 月新乡市妇幼保健院收治的 116 例PCOS不孕症患者为研究对象.按照治疗方式,将其分为观察组(n=60,采取枸橼酸氯米芬与HMG联合治疗)和对照组(n=56,仅采取枸橼酸氯米芬治疗).比较两组患者临床疗效,子宫内膜厚度,促排卵结局,性激素[血清雌二醇、孕激素、黄体生成素(LH)、卵泡雌激素(FSH)]水平以及不良反应发生情况.结果:治疗后,观察组患者总有效率,子宫内膜厚度及血清雌二醇、孕激素、FSH、LH水平,均高于对照组,差异均有统计学意义(P<0.05).观察组患者排卵率、妊娠率高于对照组,自然流产率低于对照组,差异均有统计学意义(P<0.05).两组患者不良反应发生率比较,差异无统计学意义(P>0.05).结论:将HMG与枸橼酸氯米芬联合应用于PCOS不孕患者临床疗效明显,能够显著改善子宫内膜厚度及性激素水平,促进排卵和妊娠,且安全性高.Objective:To explore the clinical ficacy of injectable human menopausal gonadotropin(HMG)combined with clomiphene citrate in patients with infertility caused by polycystic ovary syndrome(PCOS).Methods:A total of 116 PCOS infertility patients in Maternal and Child Health Hospital of Xinxiang from January 2022 to December 2023 were selected as the research objects.They were divided into observation group(n=60,treated with clomiphene citrate combined with HMG)and control group(n=56,treated with clomiphene citrate only).The efficacy,endometrial thickness,ovulation outcomes,changes in serum levels of estradiol,progestogens,luteinizing hormone(LH),follicle-stimulating hormone(FSH),and incidence of adverse reactions were compared between the two groups.Results:After treatment,the overall effective rate,endometrial thickness,and levels of estradiol,progestogens,LH and FSH of observation group were higher than those of control group,and the differences were statistically significant(P<0.05).Observation group also had higher ovulation and pregnancy rates,as well as a lower spontaneous abortion rate,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of HMG and clomiphene citrate in infertile patients with PCOS has obvious clinical ficacy,which can significantly improve endometrial thickness and female hormone levels,promote ovulation and pregnancy,and has high safety.
