摘  要:目的 探讨影响儿童视网膜母细胞瘤(RB)经选择性眼动脉化疗(SOAI)预后的相关因素.方法 回顾性分析256例单眼发病眼内期RB患儿 MRI及临床资料.分析诸因素对患儿预后的影响.结果 拥有较高信号强度增强率(△SI)、与视盘较远的距离、较小的肿瘤径线、不伴种植灶或较小的种植范围、内生型生长方式、较小的视网膜剥离范围、不伴临床高危因素、玻璃体无出血及眼球无缩小患儿的预后更佳(P<0.05),肿瘤与视盘距离、临床高危因素及肿瘤生长方式是影响预后的独立因子.结论 肿瘤与视盘距离、临床高危因素及肿瘤生长方式与预后密切相关,其他诸因素是评估患儿预后的重要指标.Objective To explore the relevant prognostic factors of retinoblastoma(RB)in children after selective ophthalmic artery infusion(SOAI).Methods The MRI and clinical data of 256 children with intraocular RB in monocular disease were analyzed retrospectively.The influence of various factors on the prognosis was analyzed.Results Chidren with higher signal intensity(△SI),longer distance from optic disc,smaller tumor diameter,no implant focus or small implant area,endogenous growth pattern,smaller retinal exfoliation area,no clinical risk factors,no vitreous hemorrhage,no eyeball shrinkage had better prognoses(P<0.05),the distance between tumor and optic disc,clinical risk factors and tumor growth pattern were independent prognostic factors.Conclusion The distance between tumor and optic disc,clinical risk factors and tumor growth pattern are closely related to the prognosis of children.Other factors can be used as important indicators to evaluate the prognosis of children.
