全髋关节置换术与闭合复位内固定术治疗创伤性股骨头缺血性坏死的效果对比

摘　　要：目的:比较全髋关节置换术(THA)与闭合复位内固定术(CRIF)治疗创伤性股骨头缺血性坏死患者的效果。方法:回顾性分析2023年1月—2024年1月湖北省第三人民医院收治的100例创伤性股骨头缺血性坏死患者资料,根据手术方法的不同将其分为对照组和研究组,各50例。对照组采用CRIF治疗,研究组采用THA治疗。对比两组治疗效果、并发症发生情况、术中出血量、疼痛程度、生活质量、髋关节功能。结果:研究组治疗总有效率为98.00%,高于对照组的84.00%,差异有统计学意义(P<0.05)。研究组术中出血量少于对照组,差异有统计学意义(P<0.05)。术后1个月,研究组疼痛视觉模拟评分低于对照组,改良Barthel指数、Harris髋关节功能量表中各个维度评分及总分均高于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为2.00%,低于对照组的16.00%,差异有统计学意义(P<0.05)。结论:THA治疗创伤性股骨头缺血性坏死患者的效果及安全性良好,可降低患者的术中出血量,缓解疼痛程度,改善髋关节功能,提高术后生活质量。Objective:To compare the efficacy of total hip arthreplasty(THA)and closed reduction and intornal fixation(CRIF)in the treatment of traumatic avascular necrosis of the femoral head.Methods:The data of 100 patients with traumatic avascular necrosis of the femoral head admitted to the Third People’s Hospital of Hubei Province from January 2023 to January 2024 were retrospectively analyzed.According to the different surgical methods,they were divided into a control group and a study group,with 50 cases in each group.The control group was treated with CRIF and the study group was treated with THA.The treatment effect,coccurrence of omplications,intraoperative blood loss,pain degree,quality of life and hip function were compared between the two groups.Results:The total effective rate of the study group was 98.00%,which was higher than 84.00%of the control group,the difference was statistically significant(P<0.05).The amount of blood loss in the study group was less than that in the control group,the difference was statistically significant(P<0.05).One month after surgery,the Pain Visual Analog Scale score of the study group was lower than that of the control group,and the scores of all dimensions and total scores of the Modified Barthel Index and Harris Hip Scale were higher than those of the control group,with statistical significance(P<0.05).The complication rate of the study group was 2.00%,which was lower than 16.00%of the control group,the difference was statistically significant(P<0.05).Conclusion:THA is effective and safe in the treatment of traumatic avascular necrosis of the femoral head,which can reduce the amount of intraoperative blood loss,relieve pain,improve hip function and improve postoperative quality of life.
