摘 要:目的分析老年人群骨折流行病学特征,为制定有效防治措施提供科学参考。方法收集2019年1月1日—2023年12月31日入院的60岁以上骨折患者的住院病案首页信息,经t检验和χ2检验分析比较骨折患者性别、年龄、临床特征、时间分布及损伤原因特点。结果研究共纳入骨折住院患者17568例,其中男性5209例(29.65%),女性12359例(70.35%),男女比为1:2.37;整体年龄中位数为71(65,81)岁,女性年龄中位数为72(66,81)岁,略高于男性年龄中位数70(64,80)岁,骨折好发年龄段主要集中在低龄老年组(60~74岁)(59.49%),其中女性比例高于男性(68.76%vs31.24%)。骨折发生人数整体随年份增加呈上升趋势,在夏秋季高发。骨折部位以髋和大腿为主(53.19%),不同性别及年龄组在骨折部位构成上存在统计学差异(χ_(性别)^(2)=372.602,χ_(年龄)^(2)=3736.827,P<0.001);骨折主要损伤原因为跌倒,不同性别及年龄组在损伤原因构成上存在统计学差异(χ_(性别)^(2)=356.889,χ_(年龄)^(2)=788.959,P<0.001)。结论老年骨折患者在性别、年龄、发生季节、骨折部位及损伤原因的分布上,均存在显著差异。应对老年骨折患者提出有针对性的预防策略,以减少其骨折的发生。Objectives This study aims to analyze the epidemiological characteristics of fracture in the elderly population,so as to provide scientific references for the development of effective prevention and treatment measures.Methods The medical record front-page information of patients aged 60 years and above who were admitted to the hospital with fractures from January 1,2019 to December 31,2023 was collected,and the gender,age,clinical characteristics,time distribution and injury cause characteristics of fracture patients were analyzed and compared.Results A total of 17,568 inpatients with fractures were included,including 5209 males(29.65%)and 12359 females(70.35%),with a male to female ratio of 1:2.37.The overall median age was 71(65,81)years old,and the median age of women was 72(66,81)years old,slightly higher than the median age of men of 70(64,80)years old.The age group with the highest incidence of fractures was mainly concentrated in the younger elderly group(60-74 years old)(59.49%),among which the proportion of women was higher than that of men(68.76%vs 31.24%).The number of fractures increased with the year,with the highest incidence in summer and autumn.The main fracture sites were hip and thigh(53.19%),and there were statistical differences in the composition of fracture sites among different genders and age groups(χ_(sex)^(2)=372.602,χ_(age)^(2)=3736.827,P<0.001).The main cause of fracture was fall,and there were statistical differences in the cause of fracture by gender and age(χ_(sex)^(2)=356.889,χ_(age)^(2)=788.959,P<0.001).Conclusions There were significant differences in gender,age,occurrence season,fracture site and injury causes in elderly patients with fracture.Targeted prevention strategies should be proposed for elderly patients with fractures to reduce the incidence of fractures.
