755例全子宫切除手术患者住院费用影响因素分析

摘　　要：目的 分析全子宫切除手术患者的住院费用情况及影响因素,为合理降低全子宫切除手术患者的住院费用提供参考依据。方法 从某三甲医院的医院信息系统采集2023年1月1日-2023年12月31日住院行全子宫切除手术的患者信息,经筛选后最终收集到755例行全子宫切除手术的患者信息,采用灰色关联分析法和多元线性回归分析法对数据进行分析。结果 子宫切除手术患者的平均费用为18 417.59元,手术费与总费用的关联度最高,其次为药品费用和护理费。筛选出对住院总费用的影响有统计学意义的七个变量,包括患者年龄、住院天数、CMI值、疾病类型、手术入路、手术时长和术前住院天数。其中,患者年龄、住院天数、CMI值、手术时长和术前住院天数对总费用造成正面影响,疾病类型和手术入路产生负面影响。结论 子宫切除手术患者的住院费用受患者年龄、住院天数、CMI值等多个因素的影响,医院管理者应当从重点影响因素入手,采取科学措施合理降低住院费用。Objectives This study aims to analyze the hospitalization expenses and influencing factors of patients undergoing total hysterectomy surgery, so as to provide a reference for reasonably reducing the hospitalization expenses of patients undergoing total hysterectomy surgery. Methods The information of patients who were hospitalized for total hysterectomy from January 1, 2023 to December 31, 2023 was collected from the hospital information system of a tertiary hospital. After screening, the information of 755 patients who underwent total hysterectomy was finally collected, and the data were analyzed using grey correlation analysis and multivariate linear regression analysis. Results The average cost for patients undergoing hysterectomy was 18,417.59 yuan. The correlation between surgical costs and total costs was the highest, followed by medication and nursing costs. Through multiple linear regression analysis, seven variables with statistically significant effects on total hospitalization costs were ultimately selected, including patient age, length of hospital stay, CMI value, disease type, surgical approach, surgical duration, and preoperative length of hospital stay. Among them, patient age, length of hospital stays, CMI value, surgical duration, and preoperative hospital stay had a positive impact on the total cost, while disease type and surgical approach had a negative impact. Conclusions The hospitalization costs of patients undergoing hysterectomy are affected by multiple factors, including patient age, length of hospital stay, and CMI value. Hospital managers should start with the key influencing factors and take scientific measures to reasonably reduce hospitalization costs.
