摘 要:目的 对胰十二指肠切除患者进行耐受性评估,以期达到改善患者营养水平,降低不耐受风险,为临床应用提供数据支持。方法 选取2023年2月1日-2024年1月31日某院70例接受胰十二指肠切除术治疗的患者,随机分为对照组与研究组,各35例;对照组接受常规管理,研究组接受基于耐受性评估的肠内营养支持。比较两组的白蛋白、肠脂肪酸结合蛋白、患者提供的主观整体营养状况评分量表评分、D-乳酸、运铁蛋白。结果 干预后,研究组的肠-脂肪酸结合蛋白、患者提供的主管整体营养状况评分、D-乳酸低于对照组,差异具有统计学意义(P<0.05),研究组的白蛋白、运铁蛋白高于对照组,差异具有统计学意义(P<0.05)。结论 肠内营养耐受性评估能够减轻胰十二指肠切术后患者胃肠功能损伤,改善患者营养水平,降低不耐受风险。Objectives This study aims to conduct a tolerance assessment on patients undergoing pancreaticoduodenectomy in order to improve the nutritional level of patients, reduce the risk of intolerance, and provide data support for clinical applications. Methods A total of 70 patients who underwent pancreaticoduodenectomy in a hospital from February 1, 2023 to January 31, 2024 were randomly divided into a control group and a study group,with 35 patients in each group. The control group received routine management, and the study group received enteral nutrition support based on tolerance assessment. The levels of albumin, intestinal fatty acid binding protein, the subjective overall nutritional status rating scale provided by the patients, D-lactate, and transferrin were compared between the two groups. Results After the intervention, the intestinal fatty acid binding protein, the overall nutritional status score provided by the patients, and D-lactic acid in the study group were lower than those in the control group,and the differences were statistically significant(P< 0.05). The levels of albumin and transferrin in the study group were higher than those in the control group, and the difference was statistically significant(P<0.05). Conclusions Enteral nutrition tolerance assessment can alleviate gastrointestinal function damage in patients after pancreaticoduodenectomy, improve patients' nutritional levels, and reduce the risk of intolerance.
