肩峰下注射富血小板血浆联合玻璃酸钠修复肩袖损伤的效果

摘　　要：目的 探讨肩峰下注射富血小板血浆(PRP)联合玻璃酸钠修复肩袖损伤的疗效。方法 选取郑州人民医院骨科2020年5月至2021年5月收治的肩袖损伤患者60例,按照随机数字表法分为PRP组(30例)和对照组(30例)。PRP组接受肩峰下注射5 mL PRP联合2.5 mL玻璃酸钠注射液,对照组接受注射2.5 mL玻璃酸钠注射液和5 mL生理盐水。两组均采用相同品牌的PRP制作设备,术后采用相同的康复方案。记录患者治疗前和治疗后1个月的视觉模拟评分(VAS)、欧洲肩关节协会的Constant肩关节评分系统评分(CMS)、美国肩肘外科医师学会(ASES)评分。结果 两组一般资料差异无统计学意义(P>0.05)。PRP组1例患者因拒绝来院复查而失访,对照组均获得随访。治疗前两组患者的VAS评分、CMS评分、ASES评分差异无统计学意义(P>0.05)。PRP组治疗后1个月VAS评分较治疗前降低,CMS评分较治疗前升高,ASES评分较治疗前升高(P<0.05)。对照组治疗后1个月VAS评分较治疗前降低,CMS评分较治疗前升高,ASES评分较治疗前升高(P>0.05)。结论 PRP联合玻璃酸钠用于修复肩袖损伤有确切的疗效,能够缓解患者的疼痛,改善患者功能,促进肩袖修复,并且操作简单方便。Objective To investigate the clinical efficacy of platelet rich plasma(PRP)combined with sodium hyaluronate in repair of rotator cuff injury.Methods Sixty patients diagnosed with rotator cuff injury in the Department of Orthopedics of Zhengzhou People’s Hospital from May 2020 to May 2021 were selected and divided into PRP group(30 cases)and control group(30 cases)according to random number table method.The PRP group received subacromial injection of 5 mL PRP combined with 2.5 mL sodium hyaluronate,and the control group received 2.5 mL sodium hyaluronate injection and 5 mL of normal saline.The same brand of PRP production equipment was used in both groups,and the same postoperative rehabilitation program was used to record the visual analogue scale(VAS),Constant-Murley score(CMS)of the European shoulder association,American shoulder and elbow surgeons(ASES)score before treatment and one month after treatment.Results There was no significant difference in general data between the two groups(P>0.05).One patient in the PRP group was lost to follow-up due to refusal to come to the hospital for reexamination.There was no significant difference in VAS score,CMS score and ASES score between the two groups before treatment(P>0.05).The VAS score decreased,CMS score increased,and ASES score increased in the PRP group 1 month after treatment compared to before treatment(P<0.05).The VAS score decreased,CMS score increased,and ASES score increased in the control group one month after treatment compared to before treatment(P>0.05).Conclusion PRP combined with sodium hyaluronate has an effective effect on the repair of rotator cuff injury.It can relieve the pain,improve the function of patients and promote the repair of rotator cuff.The operation is simple and convenient.
