摘 要:目的 探讨间隙平衡技术与测量截骨技术在重度膝骨关节炎(KOA)患者全膝关节置换术中的应用效果比较。方法 选取2021年1月至2023年3月河南大学附属郑州颐和医院收治的102例重度KOA患者作为研究对象,随机数字表法分为A组(51例)和B组(51例)。A组使用间隙平衡技术治疗,B组使用测量截骨技术治疗。比较两组患者围手术期指标、截骨量、内外侧胫骨关节角及并发症发生率。结果 A组手术时间短于B组,术中出血量少于B组(P<0.001);术后6个月时,A组内外侧屈膝90°应力位低于B组(P<0.001);两组间胫骨平台、股骨内侧与外侧后髁截骨量、术后6个月内并发症差异无统计学意义(P>0.05)。结论 全膝关节置换术中采用间隙平衡技术可缩短重度KOA患者手术时间,减少术中出血量,减轻术后内外侧屈膝90°应力位。Objective To compare of ation effects of space balance technique and measurement osteotomy technique in total knee arthroplasty for severe knee osteoarthritis(KOA)patients.Methods A total of 102 patients with severe KOA admitted to Zhengzhou Yihe Hospital Affiliated to Henan University were enrolled as the research objects from January 2021 to March 2023.According to random number table method,they were divided into group A(51 cases)and group B(51 cases).Group A was treated with space balance technique,while group B was treated with measurement osteotomy technique.The perioperative indexes,osteotomy volume,angles of medial and lateral tibial joints and incidence of complications were compared between the two groups.Results The operation time of group A was shorter than that of group B,and the intraoperative blood loss was less than that of group B(P<0.001).At 6 months after surgery,stress level of 90°medial and lateral knee bending in group A was lower than that in group B(P<0.001).There was no difference in osteotomy volume of tibial plateau,medial femur and lateral hypocondyle,and incidence of complications within 6 months after surgery between the two groups(P>0.05).Conclusion Space balance technique can shorten operation time,reduce intraoperative blood loss and relieve postoperative stress level of 90°medial and lateral knee bending in patients with severe KOA during total knee arthroplasty.
