摘  要:目的 分析医院综合重症加强护理病房(ICU)肺炎克雷伯杆菌感染状况及其耐药性。方法 回顾性分析,采集2020年1月至2022年12月信阳市中心医院综合ICU送检标本,标本检验结果资料均保存完整,统计肺炎克雷伯杆菌检出情况、标本分布情况以及药敏试验结果。结果 本次共收集到综合ICU患者1684份临床标本,检出肺炎克雷伯杆菌401株,检出率为23.81%,3 a检出率逐年升高,差异有统计学意义(P<0.05)。本次检出的肺炎克雷伯杆菌主要来源于痰标本,占比为73.82%,其次为血、腹水,分别占比为10.22%、7.48%。药敏试验结果表明,2020—2022年综合ICU检出的肺炎克雷伯杆菌耐药率整体逐年降低,并且对多种抗菌药均有不同程度的耐药,其中耐药率最高的为氨苄西林,范围在95.45%~82.14%,其次为妥布霉素、氨苄西林/舒巴坦、头孢噻肟、头孢曲松、复方磺胺甲噁唑,范围在25.27%~64.29%;肺炎克雷伯杆菌对替加环素耐药率最低,耐药率在2.20%~3.03%,其次为厄他培南、阿米卡星、亚胺培南、美罗培南、头孢哌酮/舒巴坦、头孢西丁,范围在4.55%~17.68%。结论 信阳市中心医院综合ICU在2020—2022年间肺炎克雷伯杆菌感染情况逐年增加,对常规抗菌药耐药率均有不同程度的耐药性,但整体的耐药率逐年降低,应加强耐药菌监测,以及抗菌药物的合理使用。Objective To analyze the infection status and drug resistance of Klebsiella pneumoniae in the intensive care unit(ICU)of a hospital.Methods Retrospective analysis was conducted to collect specimens from a comprehensive ICU of Xinyang Central Hospital from January 2020 to December 2022.The test results of the specimens were well preserved,and the detection of Klebsiella pneumoniae,distribution of specimens,and drug sensitivity test results were statistically analyzed.Results A total of 1684 clinical specimens were collected from ICU patients,and 401 strains of Klebsiella pneumoniae were detected,with a detection rate of 23.81%.The three-year detection rate has been increasing year by year,and the difference was statistically significant(P<0.05).The Klebsiella pneumoniae detected this time mainly came from sputum samples,accounting for 73.82%,followed by blood and ascites,accounting for 10.22%and 7.48%,respectively.The results of drug sensitivity tests showed that from 2020 to 2022,the overall resistance rate of Klebsiella pneumoniae detected in the comprehensive ICU decreased year by year,and it showed varying degrees of resistance to multiple antibiotics.Among them,ampicillin had the highest resistance rate,ranging from 95.45%to 82.14%,followed by tobramycin,ampicillin/sulbactam,cefotaxime,ceftriaxone,and compound sulfamethoxazole,ranging from 25.27%to 64.29%.Klebsiella pneumoniae had the lowest resistance rate to tigecycline,ranging from 2.20%to 3.03%,followed by ertapenem,amikacin,imipenem,meropenem,cefoperazone/sulbactam,and cefoxitin,with a range of 4.55%to 17.68%.Conclusion The infection rate of Klebsiella pneumoniae in a comprehensive ICU of Xinyang Central Hospital has been increasing year by year from 2020 to 2022,with varying degrees of resistance to conventional antibiotics.However,the overall resistance rate has been decreasing year by year.It should strengthen the monitoring of resistant bacteria and the rational use of antibiotics.
