摘 要:目的 探讨胞磷胆碱联合高频次高压氧治疗一氧化碳(CO)中毒对患者脑血流动力学的影响。方法 回顾性采集2020年1月至2022年12月濮阳市人民医院收治的50例经高频次高压氧治疗的CO中毒患者的资料,纳入对照组;同期选取50例经胞磷胆碱联合高频次高压氧治疗的CO中毒患者资料,纳入观察组,两组均完成4周治疗。比较脑血流动力学指标[颈内动脉收缩期最大流速(PSV)、平均流速(MV)、阻力指数(RI)]、认知功能[采用简明精神状态检查量表(MMSE)]。统计治疗期间不良反应发生情况。结果 治疗4周,两组PSV、MV较治疗前提高,RI较治疗前降低,且与对照组比较,观察组PSV、MV高,RI低(P<0.05);治疗4周,两组MMSE评分较治疗前提高,且与对照组比较,观察组高(P<0.05)。两组治疗期间不良反应发生率差异无统计学意义(P>0.05)。结论 胞磷胆碱联合高频次高压氧治疗可改善CO中毒患者脑血流动力学,降低神经损伤标志物水平,促进患者认知功能恢复,且不增加不良反应。Objective To explore the effect of combination therapy with cytidine and high-frequency hyperbaric oxygen on cerebral hemodynamics in patients with carbon monoxide(CO)poisoning.Methods Retrospective collection of data from 50 patients with CO poisoning treated with high-frequency hyperbaric oxygen therapy at Puyang People’s Hospital from January 2020 to December 2022,and inclusion in the control group.Fifty patients with CO poisoning treated with phosphatidylcholine combined with high-frequency hyperbaric oxygen therapy were selected during the same period and included in the observation group.Both groups completed 4 weeks of treatment.The cerebral hemodynamic indicators[peak systolic velocity(PSV),mean velocity(MV),resistance index(RI)of the internal carotid artery]and cognitive function[using the mini-mental state examination(MMSE)]were compared.The occurrence of adverse reactions during the treatment period was record.Results After 4 weeks of treatment,the PSV and MV of both groups increased compared to before treatment,while the RI decreased.Compared with the control group,the observation group had higher PSV and MV,but lower RI(P<0.05).After 4 weeks of treatment,MMSE scores in both groups were higher than before treatment,and higher in the observation group than in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two treatment groups(P>0.05).Conclusion Cytidine combined with high-frequency hyperbaric oxygen therapy can improve cerebral hemodynamics,reduce levels of nerve injury markers,promote cognitive function recovery in patients with CO poisoning,and do not increase adverse reactions.
