揉筋正骨联合平衡针刀治疗椎动脉型颈椎病的效果

摘　　要：目的 探讨揉筋正骨联合平衡针刀治疗椎动脉型颈椎病的效果。方法 前瞻性随机对照纳入河南省洛阳正骨医院2021年6月至2022年12月收治的86例椎动脉型颈椎病患者,以随机数字表法把患者分成两组,每组43例。对照组接受平衡针刀治疗,观察组在对照组基础上联合揉筋正骨中医手法治疗,治疗时间为1个月。治疗后,评估对比两组疗效。比较两组颈椎两侧动脉血流参数,包括平均血流速度(V_(mean))、血流阻力指数(RI)、动脉收缩期最大血流速度(PSV)。采用视觉模拟评分法(VAS)评估两组颈椎疼痛程度。结果 观察组治疗总有效率高于对照组(P<0.05)。治疗后两组V_(mean)、PSV水平升高,观察组高于对照组(P<0.05);RI水平降低,观察组低于对照组(P<0.05);治疗后,两组VAS评分持续下降,观察组评分均低于对照组(P<0.05)。结论 揉筋正骨联合平衡针刀可改善椎动脉型颈椎病患者的血流情况,减轻其疼痛程度,治疗整体效果较好。Objective To explore the therapeutic effect of kneading tendons and setting bones combined with balanced needle knife therapy on cervical spondylosis of vertebral artery type.Methods A prospective randomized controlled study was conducted on 86 patients with cervical spondylosis of vertebral artery type admitted to Luoyang Orthopedic-Traumatological Hospital of Henan Province from June 2021 to December 2022.The patients were divided into two groups using random number table method,with 43 patients in each group.The control group was treated with balanced needle knife therapy,while the observation group was treated with traditional Chinese medicine techniques of kneading tendons and setting bones on the basis of the control group.The treatment period was one month.After treatment,the efficacy of the two groups was evaluated and compared.The blood flow parameters of the bilateral cervical arteries of the two groups were detected,including the average blood flow velocity(V_(mean)),blood flow resistance index(RI),and maximum systolic blood flow velocity(PSV)of the arteries.The visual analogue scale(VAS)was used to evaluate the degree of cervical pain in both groups.Results The total effective rate of observation group was higher than control group(P<0.05).Before treatment,there was no statistical difference in V_(mean),RI,and PSV between the two groups(P>0.05).After treatment,the levels of V_(mean) and PSV in the two groups increased,and the observation group was higher than the control group,the RI levels in the two groups decreased,and the observation group was lower than the control group(P<0.05).After treatment,the VAS scores of the two groups continued to decrease,and observation group had lower evaluation scores than control group(P<0.05).Conclusion The combination of kneading tendons and setting bones with balanced needle knife can improve the blood flow and alleviate pain in patients with vertebral artery type cervical spondylosis,and the overall treatment effect is good.
