仅吻合单根动脉的指尖再植术后指体功能分析

摘　　要：目的 比较仅吻合单根动脉的指尖再植与常规再植指体的术后功能。方法 选择2018年1月至2023年6月河南科技大学第一附属医院收治的60例指尖离断再植患者为研究对象,按照随机数字表法分为观察组与对照组,每组30例。观察组仅吻合一根指动脉或其终末支,对照组常规吻合指动脉、指静脉及指神经。比较两组患者术后治疗效果、再植体成活率、手术时间、换药次数、住院时间、治疗费用、指体的两点辨别觉、指甲生长长度及指间关节活动度。结果 观察组再植体术后优良率与对照组比较,差异未见统计学意义(Z=0.738,P=0.460)。观察组再植体成活率略低于对照组,但差异未见统计学意义(χ^(2)=0.351,P=0.554)。观察组手术时间、治疗费用均低于对照组,差异有统计学意义(t=12.392、17.420,P<0.05)。观察组换药次数、住院时间高于对照组,差异有统计学意义(t=7.667、7.021,P<0.05)。术后6个月时,两组再植指尖的两点辨别觉距离、损指指甲生长长度、指间关节活动度比较,差异未见统计学意义(t=0.549、0.743、0.124,P=0.585)。结论 仅吻合单根动脉的指尖再植与常规指尖再植比较虽然增加了住院时间及换药次数,但手术时间更短、治疗费用更低,且在再植体优良率和成活率、两点辨别觉、损指指甲长度及指间关节活动度方面无明显差异。Objective To compare the postoperative function of fingertip replantation only anastomosed with a single digital artery and conventional replantation.Methods Sixty patients with fingertip amputation and replantation admitted from January 2018 to June 2023 were selected as the study objects.They were randomly divided into observation group and control group,with 30 cases in each group.Only one digital artery or its terminal branch was anastomosed in the observation group,while the digital artery,vein and nerve were anastomosed in the control group.The postoperative therapeutic effect,survival rate of replantation,operation time,dressing times,hospital stay,treatment cost,twopoint discernibility of finger body,fingernail growth length and interphalangeal joint motion were compared between the two groups.Results There was no significant difference in the excellent and good rate after replantation between the observation group and the control group(Z=0.738,P=0.460).The survival rate of replantation in the observation group was slightly lower than that in the control group,but the difference was not significant(x^(2)=0.351,P=0.554).The operation time and treatment cost in the observation group were lower than those in the control group,and the differences were significant(t=12.392,17.420,P<0.05).The number of dressing changes and length of hospital stay in observation group were higher than those in control group,and the differences were significant(t=7.667,7.021,P<0.05).At 6 months after surgery,there was no significance in the two-point discerning distance of fingertip replantation,the growth length of damaged fingernail,the range of motion of interphalangeal joint(t=0.549,0.743,0.124,P=0.585).Conclusions Compared with conventional fingertip replantation,fingertip replantation only anastomosed with a single digital artery increased the length of hospital stay and the number of dressing changes,but the operation time was shorter and the treatment cost was lower,and there was no significant difference between the obse
