摘 要:目的 探讨计算机断层扫描(CT)演变特征联合超敏C-反应蛋白水平评估病毒性肺炎预后的价值。方法 回顾性分析2022年1月至2024年1月在鹤壁煤业集团总医院接受治疗的72例病毒性肺炎患者的临床资料。每例患者于确诊至恢复期间分别复查5次CT,并检测超敏C-反应蛋白水平。用连续性折线图表示各时间点CT评分、超敏C-反应蛋白水平及病灶数目的动态变化。将CT评分、超敏C-反应蛋白及病灶数目进行相关性分析。结果 患者不同时点CT评分、超敏C-反应蛋白水平及病灶数目比较,差异有统计学意义(F=110.596、86.729、82.431,P<0.005)。患者CT评分呈先增后减趋势,5~10 d时CT评分最高。超敏C-反应蛋白水平呈先增后减趋势,5~10 d时超敏C-反应蛋白水平最高。病灶数目呈逐渐减少趋势,20 d后病灶数目最少。CT评分、超敏C-反应蛋白水平与病灶数目呈显著相关性(P<0.005),且治疗前后CT评分差值与5~10 d全肺病灶数目、20 d后全肺病灶数目呈显著正相关(P<0.005),治疗前后超敏C-反应蛋白差值与治疗前后病灶数目差值、0~5 d全肺病灶数目、5~10 d全肺病灶数目呈显著正相关(P<0.005)。结论 CT演变特征联合超敏C-反应蛋白水平对病毒性肺炎的预后具有重要的评估价值。Objective To investigate the value of CT evolution characteristics combined with level of high-sensitivity C-reactive protein(hs-CRP)in evaluating the prognosis of viral pneumonia.Methods The clinical data of 72 patients with viral pneumonia who were admitted and treated in Hebi Coal Industry Group General Hospital from January 2022 to January 2024 were retrospectively analyzed.Each patient underwent five CT scans and hs-CRP assessments at different time points from diagnosis to recovery.The dynamic changes of CT scores,hs-CRP,and lesion numbers at each time point were represented by continuous line graphs.Correlation analysis was performed on CT scores,hs-CRP,and lesion numbers.Results There were significant differences in CT scores,hs-CRP and number of lesions at different time points(F=110.596,86.729,82.431,P<0.005).The CT score of the patients increased first and then decreased,and the CT score was the highest at 5-10 days.The hs-CRP increased first and then decreased,and the hs-CRP level was the highest at 5~10 days.The number of lesions showed a gradual decreasing trend,and the number of lesions was the least after 20 days.CT score and hs-CRP level were significantly correlated with the number of lesions(P<0.005),and the difference of CT score before and after treatment was significantly positively correlated with the number of total lung lesions 20 days after treatment,and the number of total lung lesions 5~10 days after treatment(P<0.005).The difference of hs-CRP before and after treatment was significantly positively correlated with the difference of the number of lesions,the number of lung lesions on O~5 days and the number of lung lesions on 5~10 days(P<0.005).Conclusions The combination of CT evolution characteristics and high-sensitivity C-reactive protein has important application value in evaluating the prognostic effect of viral pneumonia.
