摘 要:目的观察远程中西医结合肺康复指导训练对病毒性肺炎患者肺功能及运动耐力影响。方法纳入2021年3月—2024年7月医院符合诊断标准的病毒性肺炎患者40例,轻度运动耐力下降组15例,中度运动耐力下降组15例,重度运动耐力下降组10例。给予远程中西医结合肺康复指导训练1个月,治疗方案包括现代呼吸康复疗法和中医传统康复疗法治疗。康复前后检测不同组别患者心率、血氧饱和度、6 min步行实验、肺功能指标、Borg呼吸困难程度和Borg主观疲劳程度。肺功能包括1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、1秒率(FEV_(1)/FVC)、呼气峰值流速(PEF)。以评估患者心肺功能及运动耐力改善情况。结果轻中度运动耐力下降组患者在远程肺康复治疗后血氧上升、心率下降、6 min步行距离增加(P<0.05)且肺功能中FEV_(1)、FEV_(1)/FVC、PEF指标上升(P<0.05);重度运动耐力下降组患者在远程肺康复治疗后血氧、心率、6 min步行距离、肺功能指标较康复前有所改善,但差异无统计学意义(P>0.05)。轻中度运动耐力下降组患者在远程肺康复治疗后Borg呼吸困难程度和Borg主观疲劳程度均有所改善(P<0.05),重度运动耐力下降组患者Borg呼吸困难程度和Borg主观疲劳程度有所改善,但差异无统计学意义(P>0.05)。结论远程中西医结合肺康复指导训练能够改善轻中度运动耐力下降病毒性肺炎患者心肺功能及运动耐力,提升生活质量。Objective To observe the effect of remote pulmonary rehabilitation guidance training of integrated traditional Chinese and western medicine on lung function and exercise endurance of patients with viral pneumonia.Methods Forty patients with viral pneumonia discharged from the Second Staff Hospital of Wuhan Huarun Wugang were divided into mild,moderate and severe exercise endurance decline groups,and were given long-distance lung rehabilitation training with integrated traditional Chinese and western medicine for one month.The treatment program included modern respiratory rehabilitation therapy and traditional Chinese medicine rehabilitation therapy.Before and after rehabilitation,heart rate,blood oxygen saturation,6-minute walking test,lung function indicators,Borg's degree of difficulty breathing,and Borg's subjective fatigue were measured in different groups of patients.Pulmonary function includes forced expiratory volume in one second(FEV_(1)),forced Vital capacity(FVC),rate in one second(FEV_(1)/FVC),and peak expiratory flow rate(PEF).To evaluate the improvement of patients'cardiopulmonary function and exercise endurance.Result After remote lung rehabilitation treatment,patients in the group with mild to moderate decreased exercise endurance experienced an increase in blood oxygen,a decrease in heart rate,an increase in 6-minute walking distance(P<0.05),and an increase in lung function indicators such as FEV_(1),FEV_(1)/FVC,and PEF(P<0.05);The patients in the severely impaired exercise endurance group showed improvements in blood oxygen,heart rate,6-minute walking distance and pulmonary function after remote lung rehabilitation treatment compared to before rehabilitation,but the difference was not statistically significant(P>0.05).The patients with mild to moderate decreased exercise endurance showed improvement in Borg dyspnea and Borg subjective fatigue after remote lung rehabilitation treatment(P<0.05),while the patients with severe decreased exercise endurance showed improvement in Borg dyspnea and Borg s
