基于cAMP/BDNF通路的针刺四神聪穴联合酸枣仁汤治疗顽固性失眠的机制研究

摘　　要：目的针灸调神法联合中药复方治疗干预顽固性失眠的临床疗效,为中医治疗顽固性失眠提供理论依据。方法将就诊于平煤神马医疗集团总医院康复理疗门诊,且符合纳入标准的60例顽固性失眠随机分为观察组和对照组,每组30例。观察组采用针刺四神聪穴联合酸枣仁汤治疗,对照组予以阿普唑仑片睡前口服治疗。两组均治疗4周,观察治疗前、中、后匹兹堡睡眠质量指数(PSQI)评分变化,检测并比较患者治疗前、中、后血清中cAMP含量,BDNF基因表达水平。结果治疗后,两组PSQI评分均较治疗前降低(P<0.05),且观察组低于对照组(P<0.05)。观察组与对照组在治疗后cAMP含量、BDNF基因表达水平均有显著升高,但观察组显著高于对照组(P<0.05)。结论针刺四神聪穴联合酸枣仁汤能有效改善失眠的临床症状,其机制可能与调节cAMP/BDNF通路有关。Objective To investigate the clinical efficacy of acupuncture for regulating spirit combined with traditional Chinese medicine in the treatment of refractory insomnia,and to provide a theoretical basis for the treatment of refractory insomnia in traditional Chinese medicine.Methods Sixty patients with refractory insomnia who met the inclusion criteria from the rehabilitation and physical therapy outpatient department of Pingmei Shenma Medical Group General were randomly divided into an observation group and a control group,with 30 cases in each group.The observation group was treated with acupuncture at Sishencong acupoint(EX-HN1)combined with Suanzaoren Decoction(酸枣仁汤),and the control group was treated with oral alprazolam tablets before bedtime.Both groups were treated for 4 weeks.The changes in the Pittsburgh Sleep Quality Index(PSQI)scores before,during and after treatment were observed,and the levels of cyclic adenosine monophosphate(cAMP)and the expression levels of brain derived neurotrophic factor(BDNF)gene in the serum of patients before,during and after treatment were detected and compared.Results Scale scores:After treatment,the PSQI scores of both groups were lower than those before treatment(P<0.05),and the score of the observation group was lower than that of the control group(P<0.05).After treatment,the levels of cAMP and the expression levels of BDNF gene in the observation group and the control group increased,and those in the observation group were higher than those in the control group(P<0.05).Conclusion Acupuncture at Sishencong acupoint(EX-HN1)combined with Suanzaoren Decoction can effectively improve the clinical symptoms of insomnia,and its mechanism may be related to the regulation of the cAMP/BDNF pathway.
