摘　　要：骨折依据不同分类标准有不同的骨折类型,根据病因分为病理性骨折、创伤性骨折以及疲劳性骨折,根据骨折是否与外界相通分为闭合性骨折和开放性骨折,根据骨折形态不同分为撕脱性骨折、粉碎性骨折、凹陷性骨折等。围绕以上分类对骨折疾病编码进行探讨,对比ICD10与ICD-11编码分类体系变化。ICD-10中肿瘤性病理性骨折无法表达肿瘤病理情况及其因果关系;非肿瘤性病理性骨折无法表达骨折具体解剖部位、病因;创伤性骨折疾病分类编码与临床疾病诊断差异性较大。在ICD-11中,新增了扩展码“骨折亚型”和“侧性”等,可根据临床需求通过预组配或后组配扩展码的簇编码形式对骨折的病因、部位、形态不同详细程度的描述,同时通过簇编码的形式加强疾病间联系。在使用上,ICD-11通过内嵌浏览器的形式,为编码工作增添了便利。ICD-11的分类结构体系更能适应当下医学发展与管理需求,缩小了疾病分类和疾病诊断的差异性,为医疗、科研、教学和管理提供精确的数据信息。Fracture according to different classification criteria have different fracture types,according to the etiology is divided into pathological fracture,traumatic fracture and fatigue fracture,according to whether the fracture is communicated with the outside world is divided into closed fracture and open fracture,according to the fracture shape is divided into avulsion fracture,comminuted fracture,depressed fracture and so on.This paper mainly discusses the code of fracture disease based on the above classification,and compares the changes of ICD10 and ICD-11 coding classification system.Limited by the established coding structure system,neoplastic pathological fracture cannot express tumor pathological conditions and the causal relationship in ICD-10.The specific anatomic site and etiology of fracture could not be expressed in non-neoplastic pathological fracture.There is a great difference between classification and clinical diagnosis of traumatic fracture disease.In ICD-11,the extension code"fracture subtype"and"laterality"are added.According to the clinical needs of the hospital,the etiology,location and shape of the fracture can be described in different levels of detail through the cluster coding form of the pre-assembled or post-assembled extension code,and the link between the diseases can be strengthened through the cluster coding form.In use,ICD-11 adds convenience to coders through the form of an embedded browser.The classification structure system of ICD-11 can better adapt to the current medical development and management needs,narrow the difference between disease classification and disease diagnosis,and provide accurate data information for medical treatment,scientific research,teaching and management.
