超声剪切波弹性成像对冈上肌腱部分撕裂的诊断价值

摘  要:目的 探讨超声剪切波弹性成像对冈上肌腱部分撕裂的诊断价值,提高冈上肌腱部分撕裂的检出率,为临床诊治提供依据。方法 选择2020年1月5日-2023年8月31日期间在某院接受肩关节镜手术129例患者,按冈上肌腱撕裂情况将129例患者分为无撕裂组(37例)、部分撕裂组(51例)和全层撕裂组(41例),所有患者均接受二维超声、SWE和MRI检查,比较各组冈上肌腱(健侧和患侧)弹性模量值,分析二维超声、SWE和MRI检查对冈上肌腱全层撕裂、部分撕裂的诊断效能。结果 无撕裂组、全层撕裂组、部分撕裂组患侧冈上肌腱弹性模量值呈升高趋势(P<0.01),全层撕裂组、部分撕裂组患侧弹性模量值均高于健侧(P<0.01)。二维超声、SWE、MRI诊断冈上肌腱全层撕裂的AUC分别为0.959(95%CI:0.904~0.996)、0.960(95%CI:0.911~0.998)、0.946(95%CI:0.901~0.989),3种检查方式对冈上肌腱全层撕裂诊断效能差异均无统计学意义(P>0.05)。二维超声、SWE、MRI诊断冈上肌腱部分撕裂的AUC分别为0.782(95%CI:0.618~0.850)、0.957(95%CI:0.906~0.996)、0.779(95%CI:0.624~0.843),SWE诊断冈上肌腱部分撕裂的AUC值高于二维超声和MRI(Z=3.213、2.785,P均<0.05)。结论 SWE弹性模量值升高对冈上肌腱不全撕裂具有较高的诊断价值。Objectives To explore the diagnostic value of ultrasound shear wave elastography(SWE)for partial tear of the supraspinatus tendon,improve the detection rate of partial tear of the supraspinatus tendon,and provide a basis for clinical diagnosis and treatment.Methods 129 patients who underwent shoulder arthroscopic surgery in a certain hospital from January 5th,2020 to August 31 st,2023 were divided into no tears(37),partial tear(51)and full thickness tear(41)according to supraspinatus tendon tear,all patients underwent two-dimensional ultrasound,SWE,and MRI examinations.The elastic modulus values of the supraspinatus tendon(healthy and affected sides)in each group were compared,analysis of the diagnostic efficacy of SWE and MRI examination for full-thickness and undifferentiated tearing of the supraspinatus tendon with the examination of two-dimensional ultrasound,SWE,and MRI.Results The elastic modulus values of the supraspinatus tendon on the affected side showed an increasing trend in the non-tear group,full layer tear group,and partial tear group(P<0.01),while the elastic modulus values on the affected side were higher of the full layer tear group and partial tear group than that of the healthy side(P<0.01).The AUC of two-dimensional ultrasound,SWE,and MRI in diagnosing full-thickness tear of the supraspinatus tendon were 0.959(95%CI:0.904~0.996),0.960(95%CI:0.911~0.998),and 0.946(95%CI:0.901~0.989),respectively.There was no statistically significant difference in the diagnostic efficacy of the three examination methods for full-thickness tear of the supraspinatus tendon(P>0.05).The AUC values for partial tear of the supraspinatus tendon diagnosed by two-dimensional ultrasound,SWE,and MRI were 0.782(95%CI:0.618~0.850),0.957(95%CI:0.906~0.996),and 0.779(95%CI:0.624~0.843),respectively.The AUC values for partial tear of the supraspinatus tendon diagnosed by SWE were higher than those of two-dimensional ultrasound and MRI(Z=3.213,2.785,P<0.05).Conclusion The increase in SWE elastic modulus value had high diagnost
