钢板内固定术治疗肱骨近端骨折的优良率及对并发症的影响

摘　　要：目的分析钢板内固定术治疗肱骨近端骨折的优良率及对并发症的影响。方法选取2021年1月-2023年12月玉山博爱医院收治的肱骨近端骨折患者60例作为本研究对象,按照随机抽签法分为对照组和观察组,每组30例。对照组运用夹板外固定治疗,观察组运用钢板内固定治疗。比较两组骨折优良率、肩关节活动度(上举、前屈、后伸、外展、内收)、Constant-Murley肩关节功能评分(疼痛、活动功能、肌力、活动度)、并发症(神经损伤、血管损伤、切口感染、关节功能障碍)发生率。结果观察组骨折优良率为96.67%,高于对照组的73.33%(P<0.05)。观察组术后肩关节活动度高于对照组,疼痛、活动功能、肌力、活动度评分均高于对照组(P<0.05)。观察组并发症发生率为3.33%,低于对照组的26.67%(P<0.05)。结论钢板内固定术治疗肱骨近端骨折患者的效果确切,可以提高患者骨折优良率以及肩关节功能,降低患者术后并发症发生率,改善患者肩关节活动度,值得应用。Objective To analyze the excellent and good rate of plate internal fixation in the treatment of proximal humeral fractures and its effect on complications.Methods A total of 60 patients with proximal humeral fractures admitted to Yushan Boai Hospital from January 2021 to December 2023 were selected as the subjects of this study.According to the random lottery method,they were divided into control group and observation group,with 30 patients in each group.The control group was treated with splint external fixation,and the observation group was treated with plate internal fixation.The fracture excellent rate,shoulder range of motion,Constant-Murley shoulder function score(pain,activity function,muscle strength,activity)and complications(nerve injury,vascular injury,incision infection,joint dysfunction)were compared between the two groups.Results The fracture excellent rate of the observation group was 96.67%,which was higher than 73.33%of the control group(P<0.05).The postoperative shoulder range of motion in the observation group was higher than that in the control group,and the scores of pain,activity function,muscle strength and activity were higher than those in the control group(P<0.05).The incidence of complications in the observation group was 3.33%,which was lower than 26.67%in the control group(P<0.05).Conclusion Plate internal fixation is effective in the treatment of patients with proximal humeral fractures,which can improve the fracture excellent rate and shoulder joint function,reduce the incidence of postoperative complications,and improve the shoulder range of motion of patients.It is worthy of application.
