早期预警评分联合CRP在高脂血症性胰腺炎严重程度及预后中的临床价值

摘　　要：目的探究早期预警(NEWS)评分联合C反应蛋白(CRP)对高脂血症性胰腺炎(HLP)病情严重程度及预后的评估价值。方法纳入2020年5月-2023年7月上犹县人民医院收治的66例HLP患者,依据其病情严重程度分为轻度组(32例)、中度组(20例)、重度组(14例),检测其血清CRP水平,完成NEWS量表评分,并进行预后观察,比较轻度组、中度组、重度组的CRP水平及NEWS评分,分析CRP、NEWS评分与HLP病情严重程度的相关性,比较预后不良与预后良好患者的CRP和NEWS评分,分析CRP联合NEWS评分对HLP患者预后不良的预测价值。结果三组CRP水平及NEWS评分比较,差异有统计学意义(P<0.05),其中CRP水平及NEWS评分由低到高依次为轻度组<中度组<重度组。Spearman相关性分析显示,CRP水平、NEWS评分与HLP病情严重程度呈正相关(P<0.05)。预后不良组CRP水平、NEWS评分高于预后良好组(P<0.05)。ROC曲线显示,CRP联合NEWS评分预测HLP预后不良的AUC、敏感度、特异度大于二者单独检测(P<0.05)。结论CRP水平、NEWS评分与HLP病情严重程度存在相关性,二者联合检测对HLP预后结局具有较高的预测价值。Objective To explore the value of early warning(NEWS)score combined with C-reactive protein(CRP)in evaluating the severity and prognosis of hyperlipidemic pancreatitis(HLP).Methods A total of 66 patients with HLP admitted to Shangyou County People's Hospital from May 2020 to July 2023 were divided into mild group(32 patients),moderate group(20 patients)and severe group(14 patients)according to the severity of the disease.The serum CRP level was detected,the NEWS scale score was completed,and the prognosis was observed.The CRP level and NEWS score of mild group,moderate group and severe group were compared.The correlation between CRP and NEWS score and the severity of HLP was analyzed.The CRP and NEWS scores of patients with poor prognosis and good prognosis were compared.The predictive value of CRP combined with NEWS score for poor prognosis in patients with HLP was analyzed.Results There were statistically significant differences in CRP level and NEWS score among the three groups(P<0.05).The CRP level and NEWS score from low to high were mild group<moderate group<severe group.Spearman correlation analysis showed that CRP level and NEWS score were positively correlated with the severity of HLP(P<0.05).The CRP level and NEWS score in the poor prognosis group were higher than those in the good prognosis group(P<0.05).The ROC curve showed that the AUC,sensitivity and specificity of CRP combined with NEWS score in predicting poor prognosis of HLP were greater than those of the two alone(P<0.05).Conclusion CRP level and NEWS score are correlated with the severity of HLP,and the combined detection of the two has a high predictive value for the prognosis of HLP.
