益气养阴法治疗2型糖尿病合并血脂异常疗效的Meta分析

摘　　要：目的:系统评价益气养阴法结合西医常规治疗对2型糖尿病合并血脂异常病人的疗效及安全性。方法:检索中国知网(CNKI)、万方数据库(WanFang Data)、维普中文期刊文献服务平台(VIP)、中国生物医学文献服务系统(SinoMed)、PubMed、the Cochrane Library、荷兰医学文摘数据库(MedLine),检索时间范围为数据库创建至2023年9月,全面收集益气养阴法结合西医常规治疗(试验组)对比单纯西医常规治疗(对照组)干预2型糖尿病合并血脂异常的随机对照试验(RCTs);使用RevMan 5.3软件对提取的相关数据进行Meta分析。使用Stata MP16对提取的相关数据进行敏感性分析,并对纳入>10篇研究的结果进行发表偏倚Egger检验。结果:共纳入16项研究,涉及病人1314例。Meta分析结果显示,益气养阴法联合西医常规治疗对比单纯西医常规治疗在临床总体疗效[RR=1.29,95%CI(1.19,1.40),P<0.00001]、中医证候疗效[RR=1.34,95%CI(1.23,1.46),P<0.00001]、总胆固醇[MD=-0.42,95%CI(-0.57,-0.28),P<0.00001]、三酰甘油[MD=-0.60,95%CI(-0.77,-0.43),P<0.00001]、高密度脂蛋白胆固醇[MD=0.16,95%CI(0.08,0.15),P<0.00001]、低密度脂蛋白胆固醇[MD=-0.40,95%CI(-0.52,-0.28),P<0.00001]、空腹血糖[MD=-0.83,95%CI(-1.12,-0.54),P<0.00001]、餐后2 h血糖[MD=-1.18,95%CI(-1.62,-0.74),P<0.00001]、糖化血红蛋白[MD=-0.55,95%CI(-0.86,-0.24),P<0.00001]方面均优于单纯西医常规治疗,差异均有统计学意义(P<0.05)。所纳入文献发表偏倚较小(P>0.05),结果可靠。结论:现有证据表明,益气养阴法联合西医常规治疗在提升临床治疗总有效率,改善病人症状、血脂、血糖指标方面具有较好的效果,安全性较好,但仍需要质量高、更规范的RCTs来进一步研究分析。Objective:To evaluate the efficacy and safety of the method of tonifying Qi and nourishing Yin combined with conventional Western medicine for treating in patients with type 2 diabetes mellitus and dyslipidemia.Methods:China National Knowledge Infrastructure(CNKI),Wanfang Data,VIP Chinese Journal Literature Service Platform,China Biology Medicine Disc,PubMed database,the Cochrane Library,and the Netherlands Medical Abstracts database were searched from the establishment of the database to September 2023.A comprehensive collection of randomized controlled trials(RCTs)of the intervention of type 2 diabetes mellitus with dyslipidemia using the methool of tonifying Qi nourishing Yin combined with conventional Western medicine(observation group)versus conventional Western medicine alone(control group)was performed.RevMan 5.4 software was used for Meta-analysis of the extracted data.Stata MP16 was used to perform sensitivity analysis of the extracted data,and Egger's test for publication bias was performed in the results of more than 10 included studies.Results:A total of 16 studies were included,involving 1314 patients.The results of the Meta-analysis showed that the method of tonifying Qi and nourishing Yin combined with conventional Western medicine therapy can increase the overall clinical efficacy(RR=1.29,95%CI 1.19-1.40,P<0.00001)and Chinese medicine symptom efficacy(RR=1.34,95%CI 1.23-1.46,P<0.00001)more than that of Western medicine therapy alone.Meanwhile,the combination of Chinese and Western medicine was superior to Western medicine alone in improving total cholesterol(MD=-0.42,95%CI-0.57--0.28,P<0.00001),triglyceride(MD=-0.60,95%CI-0.77--0.43,P<0.00001),high-density lipoproteincholesterol(MD=0.16,95%CI 0.08-0.15,P<0.00001),low-density lipoprotein cholesterol(MD=-0.40,95%CI-0.52--0.28,P<0.00001),fasting plasma glucose(MD=-0.83,95%CI-1.12--0.54,P<0.00001),2 h postprandial blood glucose(MD=-1.18,95%CI-1.62--0.74,P<0.00001),and hemoglobin A1c(MD=-0.55,95%CI-0.86--0.24,P<0.00001).The differences were statistical
