清热化痰汤联合振动排痰仪治疗慢性阻塞性肺疾病急性加重期痰热雍肺证的临床研究

摘　　要：目的探讨清热化痰汤联合振动排痰仪治疗慢性阻塞性肺疾病急性加重期(AECOPD)痰热雍肺证的临床效果。方法选取2022年7月至2023年9月萍乡市中医院收治的60例AECOPD痰热雍肺证患者作为研究对象,按照随机数字表法分为对照组和试验组,每组30例。对照组给予清热化痰汤治疗,试验组在对照组基础上联合振动排痰仪治疗。比较两组患者临床疗效、炎症反应、肺功能、中医证候评分变化。结果试验组治疗后的血清白细胞计数(WBC)、C反应蛋白(CRP)中性粒细胞百分比(N%)水平低于对照组,差异有统计学意义(P<0.05)。试验组治疗后的用力肺活量(FVC)第1秒用力呼气容积(FEV)FEV,/FVC水平高于对照组,差异有统计学意义(P<0.05)。试验组治疗后的中医证候评分低于对照组,差异有统计学意义(P<0.05)。试验组治疗后的总有效率高于对照组,差异有统计学意义(P<0.05)。结论AECOPD痰热雍肺证患者采用清热化痰汤联合振动排痰仪治疗,可帮助减轻患者炎症反应,提高患者肺功能,改善患者中医症状,有助于提高疗效,可推广应用。Objective To investigate the clinical effect of Qingre Huatan Decoction combined with vibrating phlegm expec-torator in treating phlegm-heat obstructing lung syndrome of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods A total of 60 patients with phlegm-heat obstructing lung syndrome of AECOPD admitted to Pingxiang Chinese Medicine Hospital from July 2022 to September 2023 were selected as the study objects,and they were divided into control group and experimental group according to random number table method,with 30 cases in each group.The control group was treated with Qingre Huatan Decoction,and the experimental group was treated with vibrating phlegm expectorator based on the control group.The clinical efficacy,inflammatory response,lung function and traditional Chinese medicine syndrome scores were compared between the two groups.Results The serum white blood cell count(WBC),C-reactive pro-tein(CRP)and percentage of neutrophils(N%)in the experimental group after treatment were lower than those in the control group,and the dfferences were statistically significant(P<0.05).The levels of forced vital capacity(FVC),forced expiratory volume in the first second(FEV:)and FEV/FVC in the experimental group were higher than those in the control group after treatment,and the differences were statistically significant(P<0.05).The traditional Chinese medicine syndrome score of the experimental group after treatment was lower than that of the control group,and the difference was statistically significant(P<0.05).The total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion The treatment of AECOPD patients with phlegm-heat obstructing lung syn-drome by Qingre Huatan Decoction combined with vibrating phlegm expectorator can help to reduce the inflammatory response of patients,improve the lung function of patients,improve the traditional Chinese medicine syndrome,help to improve the curative effect,an
