蠲痹汤联合洛芬待因缓释片治疗寒湿痹阻型膝骨关节炎的效果

摘　　要：目的探究蠲痹汤联合洛芬待因缓释片治疗寒湿痹阻型膝骨关节炎的效果,分析其临床价值。方法选取2021年6月至2024年6月云南省中医医院收治的106例寒湿痹阻型膝骨关节炎患者为研究对象,按照随机数字表法分为对照组和试验组,每组各53例。对照组患者给予洛芬待因缓释片治疗,试验组患者在洛芬待因缓释片治疗的基础上给予蠲痹汤进行联合治疗。比较两组的临床治疗总有效率、血清炎症因子水平[白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)]、骨关节炎指数[西安大略和麦克斯特大学骨关节炎指数(WOMAC)]、疼痛程度[视觉模拟评分法(VAS)]及关节功能障碍[奎森功能演算指数(Lequesne)]变化情况。结果试验组患者的临床治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗后,试验组的IL-6、IL-1β、TNF-α水平低于对照组,差异有统计学意义(P<0.05);试验组WOMAC评分、VAS评分、Lequesne评分低于对照组,差异有统计学意义(P<0.05)。结论在洛芬待因缓释片治疗的基础上,采用蠲痹汤对寒湿痹阻型膝骨关节炎患者进行治疗,可显著提高患者临床疗效,降低患者机体炎症因子水平,缓解患者膝关节疼痛,改善患者膝关节功能障碍。Objective To explore the effect of Juanbi Decoction combined with Compound Codeine Phosphate and Ibupro-fen Sustained Release Tablets in the treatment of knee osteoarthritis with cold dampness obstruction type,and to analyze its clinical value.Methods A total of 106 patients with cold dampness obstruction type of knee osteoarthritis treated in Yunnan Provincial Hospital of Traditional Chinese Medicine from June 2021 to June 2024 were selected as the study objects,and they were divided into control group and experimental group according to random number table method,with 53 cases in each group.Patients in the control group were given Compound Codeine Phosphate and Ibuprofen Sustained Release Tablets,patients in the experimental group were treated with Juanbi Decoction on the basis of Compound Codeine Phosphate and Ibuprofen Sustained Release Tablets.The total effective rate of clinical treatment,serum inflammatory factors level(in-terleukin-6[IL-6],interleukin-1β[IL-1β],tumor necrosis factor-α[TNF-α]),osteoarthritis index(Western Ontario and McMaster Universities osteoarthritis index[WOMAC]),pain degree(visual analogue scale[VAS])and joint dysfunction(Lequesne functional index[Lequesne])were compared between the two groups.Results The total effective rate of the ex-perimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of IL-6,IL-1βand TNF-αin experimental group were lower than those in control group,and the differences were statistically significant(P<0.05).The WOMAC score,VAS score and Lequesne score of experimental group were lower than those of control group,and the differences were statistically significant(P<0.05).Conclusion On the basis of the treatment of Compound Codeine Phosphate and Ibuprofen Sustained Release Tablets,Juanbi Decoction can significantly improve the clinical efficacy,reduce the level of inflammatory factors,relieve the pain of knee joint and improve the dysfunction of knee joint.
